胃泌素、幽门螺杆菌及T细胞联合检测在老年胃炎病情发展及预后评估中的应用研究

摘　　要：目的：研究胃泌素、幽门螺杆菌（ Hp）及T细胞联合检测在老年胃炎病情发展及预后的评估中的意义,从而为老年胃炎病情发展的病情评估、疗效评价及预后判断提供帮助。方法：选择2010-2013年在我院消化内镜室胃镜检查的老年人共585人,134例患者均经胃镜检查确诊为慢性胃炎,78例患者被确诊为胃癌。这些患者在治疗前,清晨取空腹患者肘静脉血5ml,用酶联免疫吸附法进行胃泌素及血清survivin抗体检测,同时进行快速尿素酶试验检测幽门螺杆菌检测,对结果进行统计学分析。结果： Hp 检测阳性率为49.8%,伴随诊断出的十二指肠炎阳性率为13.5%、消化性溃疡为13.1%、反流性食道炎为11.3%、胃息肉为6.1%。胃癌组早期和中后期血清中survivin抗体的浓度值明显高于正常对照组,差异显著（ t值分别为2.595和6.451,P 〈0.05）；胃癌组早期和中后期血清中胃泌素的浓度值明显高于正常对照组,差异显著（ t值分别为2.981和5.891,P 〈0.05）。胃泌素及血清survivin抗体含量与病理组织中检测的结果保持一致。结论： Hp检测提高胃炎检出率,血清Survivin抗体和胃泌素含量在胃癌时明显增高,与胃癌的发生、发展和组织分化程度关系密切；联合检测有助于胃癌筛查、病情监测、疗效评价和预后判断。OBJECTIVE： To study changes of the content of gastrin Hp and survivin antibody in the serum of the elderly gastritis which can be helpful for screening, condition assessment, evaluation of curative effect and prognosis judgment of the gastric cancer. METHODS： Total 585 elderly people were diagnosed by endoscopy in 2010 to 2013 in our hospital. Among them 134 patients were chronic gastritis and 78 patients were stomach cancer. The contents of survivin antibody and GAS were detected by ELISA method. Simultaneous Helicobacter pylori was also detected. Then the results were anlysised by statistics. RESULTS： The concentrations of survivin antibody in the serum of early gastric carcinoma group and later gastric carcinoma group were obviously higher than that in the normal group （ t =2. 595 and 6. 451, P 〈0. 05）; the concentrations of gastrin in serum of early gastric carcinoma group and later gastric carcinoma group were also obviously higher than that in the normal group （ t =2. 981 and 5. 891, P 〈0. 05）, which was con-sistent with the results of the detection of pathological tissues. CONCLUSION： Hp can improve the detection of gastritis. The content of survivin antibody and GAS is significantly increased in gastric cancer. It is positively correlated between the survivin antibody and GAS and it is closely related to occurrence and development and differentiation degree of gastric cancer, which may be helpful for screening gastric cancer, monitoring condition, evaluating curative effect and judging prognosis.
