DVC免缝扎技术在腹腔镜前列腺癌根治术中的应用

摘　　要：目的：探讨阴茎背深静脉复合体（dorsal vascular complex,DVC）免缝扎技术在腹腔镜下前列腺癌根治术（laparoscopic radical prostatectomy,LRP）中的安全性和优越性。方法：回顾性分析2013年9月2014年6月在我院行LRP的13例前列腺癌患者的临床资料：7例行DVC免缝扎LRP术,为改良组;另有同期进行的6例常规LRP术,为标准组。比较两组手术时间、出血量、尿管留置时间、术后控尿率等临床指标。结果：改良组手术时间比标准组少（P〈0.05）,但出血量、尿管留置时间、术后控尿率等指标与标准组相比,差异无统计学意义（P〉0.05）。结论：DVC免缝扎技术在LRP中是安全、可行的,能减少手术时间,并在一定程度上改善术后控尿功能,且并不增加出血风险。Objective： To discuss the safety and superiority of dorsal vascular complex （DVC） ligation-free method in laparoscopic radical prostatectomy （LRP）. Method：The clinical data of 13 LRP cases were analyzed retrospectively. The improved group was seven cases undergoing LRP with DVC iigation-free method and the other six cases was normal group with standard LRP. The time of operation, amount of blood loss, indwelling time of urinary catheter and the continence rate after operation were compared between the two groups. Result：The operation time of the improved group was shorter than the standard group （P〈0.05）, but there were no significant differences in the amount of blood loss, indwelling time of urinary catheter and the continence rate after operation （P〉0.05）. Conelusion：DVC ligation-free method in LRP is safe and practicable, and it can reduce the operation time and improve postoperative continence rate to some extent with no increase in the risk of bleeding.
