溃疡性结肠炎患者外周血调节性T细胞及自然杀伤细胞比例的变化

摘　　要：目的探讨活动期溃疡性结肠炎(UC)患者外周血中调节性T(Treg)细胞、自然杀伤细胞(NK)比例的变化及其临床意义。方法检测UC患者31例(活动期17例,缓解期14例)和15名正常人外周血Treg细胞和NK细胞比例、血沉(ESR)和C反应蛋白(CRP)。结果 UC活动期患者Treg细胞比例为(4.48±1.61)%,低于缓解期的(7.10±1.39)%(P<0.05);均低于正常人的(9.61±1.87)%(P<0.05)。全结肠型UC患者Treg细胞比例低于左半结肠型[(3.41±1.38)%vs.(5.22±1.36)%](P<0.05)。UC活动期患者外周血NK细胞比例低于缓解期[(10.16±4.78)%vs.(18.42±4.96)%](P<0.05)。活动期UC患者外周血Treg细胞和NK细胞比例与疾病活动度Mayo评分呈负相关(R1=0.65,R2=0.57,P<0.05)。UC活动期ESR和CRP水平高于缓解期(P<0.05)。结论 UC外周血Treg细胞及NK细胞比例随着炎症加重而降低;检测外周血Treg细胞及NK细胞比例有助于疾病活动的评估。Objective To investigate the proportion and clinical significance of regulatory T cells（Treg cells） and Natural killer（NK） cells in peripheral blood of the patients with ulcerative colitis （UC）. Methods The proportions of Treg cells and NK cells in peripheral blood, erythrocyte sedimentation rate （ESR） and C-reactive protein（CRP） were detected in 17 cases with active UC （group A）,14 cases with UC in remission stage（group B） and 15 healthy people（group C）. Results The proportion of Treg cells in group A was lower than that in group BE（4. 48±1.61）% vs. （7.10± 1.39） %]（P〈0. 05）,which were lower than （9.61± 1.87）% in group C（P〈0. 05）. The proportion of Treg Cells was lower in the cases with pancolitis than that in those with left-side colitis1-（3. 41±1.38） % vs. （5.22±1.36）%（P〈0. 05）. The proportion of NK cells was lower in group A than that in group B[（10. 16±4.78）% vs. （18.42±4. 96）%] （P〈0. 05）. The proportions of Treg cells and NK cells in peripheral blood were negatively correlated with Mayo scores （R1 =0.65, R2 =0. 57, P〈0. 05）. The levels of ESR and CRP were higher in group A than those in group B（P〈0. 05）. Conclusion The proportions of Treg cells and NK cells in peripheral blood are decreased as UC gets severe. Detection of the proportions of Treg cells and NK cells in peripheral blood is useful in evaluating the activity of UC.
