鼻内镜手术治疗鼻腔接触性头痛的临床分析

摘　　要：目的研究由于鼻腔解剖结构异常导致鼻腔黏膜接触引起头痛的患者鼻内镜手术治疗效果。方法回顾性分析46例鼻腔接触性头痛患者,经鼻内镜检查及鼻窦CT扫描评估鼻腔解剖异常情况,并通过功能性鼻内镜手术纠正鼻腔解剖结构异常,术后3个月进行疗效评估。结果 46例患者中44例鼻内镜术后成功随访均达3～12个月,失访2例。44例中治愈32例,显效10例,无效2例。结论鼻寞CT扫描和鼻内镜检查能较准确地确定引起鼻腔接触性头痛病灶的确切部位,功能性鼻内镜手术下行鼻腔结构重建是此病有效的治疗方法。Objective To study the therapeutic effect of endoscopic sinonasal surgery to nasal mucosa contact headache which was caused by nasal anatomic abnormalities. Methods A retrospective review was performed on 46 cases that met the criteria of having contact points between the septum and the lateral nasal wall,which diagnosed by CT scans of the sinuses and the endoscopic examinations. These patients underwent surgical intervention in order to correct the contact points. They were re- evaluated for clinical parameters three months after the surgical correction of their anatomic abnormalities. Results After 3- 12 mouths following- up,32 patients were cured,10 patients were relieved to an extent and two patients were invalid. Conclusion The key causes of contact point headaches are displayed through CT scans of the sinuses and endoscopic examinations of nasal. Nasal cavity structure reconstruction is effective for cases with nasal mucosa contact headache by endoscopic sinonasal surgery.
