摘  要:目的探讨超声刀在经左胸小切口食管癌根治术中的应用价值。方法回顾性分析2010年10月至2014年5月经左胸小切口手术治疗食管癌138例的临床资料。术中应用超声刀82例为观察组;对照组56例未应用超声刀。比较两组术中及术后各项指标的差异。结果两组均顺利完成手术,观察组手术时间、术中出血量、脾破裂发生率、术后胸腔引流量、引流管留置时间均显著少于对照组(P<0.05);两组淋巴结清扫数量、肋骨骨折发生率的差异无统计学意义(P>0.05)。结论超声刀在食管癌根治术中应用可减小手术切口,缩短手术时间,减少术中出血量和术后引流量,符合微创理念,值得临床推广。Objective To investigate the application value of ultrasonically activated scalpel in esophageal carcinoma radical operation by left thoracic small incision. Methods The clinical date of 138 patients who underwent esophageal carcinoma radical operation by left thoracic small incision from 2010 October to 2014 May were analysed retrospectively.82 cases applied ultrasonically activated scalpel as the observation group,56 cases in control group were not applied. All of them received same operation. Differences in various indexes of the two groups were compared. Results The two groups were successfully completed the operation. Compared patients in the control group,the operation duration,intraoperative blood loss,splenic rupture ratio during operation,drainage volume after operation and drainage tube indwelling time were significantly reduced in the observation group,the difference was statistically significan( P 0. 05); The number of resected lymph nodes and incidence rate of rib fracture between the two groups of patients no significant difference( P〉0. 05). Conclusions Application of ultrasonically activated scalpel can reduce the operation incision,shorten operation time,reduce the bleeding and postoperative drainage volume in esophageal carcinoma radical operation by left thoracic small incision,consistent with the idea of minimally invasive surgery,is worth the clinical promotion.
