脾脏腹膜后入路行左上腹肿瘤联合脏器切除手术23例

摘　　要：目的探讨左上腹肿瘤联合脏器切除手术入路方式。方法2005年12月。2011年5月间对23例患者采用脾脏腹膜后入路对左上腹较大肿瘤行联合脏器切除，回顾性分析该手术方式平均所需手术时间、平均术中出血量、胃肠道恢复时间。结果23例采用脾脏后腹膜入路对左上腹较大肿瘤行联合脏器患者均痊愈出院。12例胰体尾＋脾脏＋左半结肠切除术平均所需手术时间108．60min；平均术中出血量209．87ml；11例胰体尾＋脾脏＋左半结肠＋近端胃切除平均所需手术时间138．50min；平均术中出血量308．64ml；23例患者胃肠恢复平均6．6d。结论脾脏后腹膜入路行左上腹肿瘤联合脏器切除手术方式安全，易行。Objective To explore the operative approach resection of left upper abdominal tumor combined with multiple organs. Methods From December 2005 to May 2011,23 patients received the resection of left upper abdominal tumor combined with multiple organs via a retroperitoneal approach behind the spleen. The mean operation time, intraoperative blood loss and gastrointestinal recovery time were reviewed retrospectively. Results All patients were successfully healed and discharged. Pancreatic body and tail, spleen, and left hemicolon were resected in 12 cases ,with a mean operation time of 108.60 min and a mean intraoperative blood loss of 209.87 ml. In the other 11 cases, the pancreatic body and tail, left hemicolon, and proximal stomach were resected, with a mean operation time of 138.50 min and a mean in- traoperative blood loss of 308.64 ml. The mean gastrointestinal recovery time was 6.6 d in all patients. Conclusion Resection of left upper abdominal tumor combined with multiple organs via a retroperitoneal approach behind the spleen is safe and simple.
