摘 要:目的探讨脯氨酸羟基化酶3(PHD3)、缺氧诱导因子-1(HIF-1)和核转录因子κB(NF-κB)在宫颈癌新辅助化疗前后癌组织中的表达分布,及其在预测肿瘤耐药性和预后中的临床意义。方法选取确诊为宫颈癌患者60例,术前均行2个疗程紫杉醇加顺铂(TP)方案化疗。应用SP免疫组织化学法检测60例患者化疗前后癌组织中PHD3、HIF-1和NF-κB的蛋白水平,并进行相关分析。结果 PHD3、NF-κB蛋白在宫颈癌组织及正常宫颈组织中均有表达分布;HIF-1在正常宫颈组织中基本无表达,而在宫颈癌组织中明显表达。在正常宫颈组织中,PHD3和NF~κB蛋白表达分布显著低于在癌组织中(x^2值分别为5.372,4.521,均P<0.05)。PHD3、HIF-1和NF-κB蛋白阳性表达与宫颈癌不同病理分级、临床分期及淋巴转移情况密切相关,差异均有统计学意义(x^2值分别为6.193、4.463、11.854,均P<0.05)。化疗后宫颈癌组织中HIF-1和NF-κB表达较化疗前均有所降低,而PHD3的表达较化疗前升高,差异均有统计学意义(x值分另为6.112、7.532,均P<0.05)。PHD3、HIE-,1和NF-κB表达阳性者与阴性者化疗有效率比较差异均有统计学意义(x^2值分别为11.726、7.205,均P<0.05)。结论 PHD3、HIF-1和NF-κB可以作为判定宫颈癌侵袭能力及预判化疗有效的指标之-,对宫颈癌的预后判断具有重要意义。Objective To investigate the expressions of prolyl hydroxylase 3 (PHD3), hypoxia inducible factor 1 (HIF-1) and nuclear factor-κB ( NF-κB ) before and after neoadjuvant chemotherapy ( NACT ) in cervical cancer tissues, and to explore their clinical significance in prediction of drug resistance and prognosis.Methods Sixty patients with cervical cancer were selected, and they underwent two courses of paclitaxel plus cisplatin chemotherapy.PHD3, HIF-1 and NF-κB were detected by SP immunohistochemical method in 60 patients before and after chemotherapy.Results PHD3 and NF-κB protein expressed in both cervical carcinoma and normal cervical tissues.HIF-1 did not expressed in normal cervical tissues, but it obviously expressed in cervical carcinoma.The expressions of PHD3 and NF-κB protein in normal cervical tissues were significantly lower than those in cancer tissues (χ^2 value was 5.372 and 4.521, respectively, both P〈0.05).The positive expressions of PHD3, HIF-1 and NF-κB were closely related to the pathological grading, clinical staging and lymph node metastasis, and the differences were significant (χ^2 value was 6.193, 4.463 and 11.854, respectively, all P〈0.05) .After chemotherapy, HIF-1 and NF-κB expressions in cervical cancer tissues decreased, but the expression of PHD3 increased with statistical significance (χ^2 value was 6.112 and 7.532, respectively, both P〈0.05).The difference in chemotherapy effective rate between patients with positive expressions of PHD3, HIF-1 and NF-κB and those with negative expressions of these three was significant (χ^2 value was 11.726 and 7.205, respectively, both P〈0.05).Conclusion PHD3, HIF-1 and NF-κB may be used as indexes to determine the invasion of cervical cancer and to prejudge the chemotherapy effect on the prognosis of cervical cancer.
