腮腺深叶肿瘤累及咽旁间隙的影像学表现

摘　　要：目的探讨累及咽旁间隙腮腺深叶来源肿瘤的影像学特点。方法回顾性分析16例腮腺深叶肿瘤的CT和MRI表现,包括多形性腺瘤5例、腺淋巴瘤5例、基底细胞腺瘤2例、鳞状细胞癌1例、黏液样表皮样癌1例、多形性腺瘤内癌2例。2例行CT、12例行MRI、2例同时完成CT、MRI检查。结果 12例肿块呈椭圆形,不规则4例。16例肿瘤与腮腺间未见脂肪间隙,6例见颈动脉间隙向后侧推移,5例见脂肪帽征,1例见腮腺蒂征,13例见肿瘤位于茎突前间隙,3例见茎突包绕。根据其定位,本组术前诊断均为腮腺深叶肿瘤,并以根据其影像特点,初步推断了肿瘤的类型,可与其他咽旁间隙肿瘤鉴别。结论累及咽旁间隙的腮腺深叶肿瘤因其发生特殊位置而具有特征性影像学征象,能协助与咽旁间隙其他肿瘤鉴别。Objective To analyze the imaging features of deep-lobe parotid tumors invading the parapharyngeal space. Methods CT （4） and MRI （14 cases） of 16 deep-lobe parotid tumorswere retrospectively analyzed. The neoplasms included pleomorphic adenomas （5 cases）, Warthin tumor （5 cases）, basal cell adenomas （2 cases）, squamous-cell carcinoma （1 case）, myxoid epidermoid carcinoma （ 1 case）, and carcinoma in pleomorphic adenoma （2 cases）. Results The lesions presented as oval （12 cases） or irregular （4 cases） masses with obliteration of fat spaces between the tumors and the deep lobe of the parotid glands. There were posterior displacement of the carotid space （6 cases）, fat cap sign （5 cases）, parotid pedicle sign （ 1 case） as well as posterior displacement of the styloids （ 13 cases） or tumor surrounding the styloids （3）. Conclusion Deep-lobe parotid tumors invading the parapharyngeal space can be distinguished from other tumors of parapharyngeal space by the location and imaging features.
