摘 要:目的:探讨眼底血管造影(fundus fluorescein angiography,FFA)图像显示存在渗漏点和频域光学相干断层扫描(spectral-domain optical coherence tomography,SD-OCT)图像显示黄斑区有局限性脱离区的急慢性中心性浆液性脉络膜视网膜病变(central serous chorioretinopathy,CSC)患者的眼底自发荧光(fundus autofluorescence,FAF)特点。方法:选择FFA图像上存在渗漏点的CSC患者19例(19眼),其中病程少于6个月的CSC患者8例(8眼),为急性CSC患者(急性组);病程大于6个月的CSC患者11例(11眼),为慢性CSC患者(慢性组)。结合SD-OCT、吲哚青绿血管造影(indocyanine green angiography,ICGA)等观察19例CSC患者的眼底FAF特点。结果:FFA图像显示出现渗漏点的视网膜区域的FAF表现为:急性组的8眼中,6眼(占75.0%)呈现低自发荧光,1眼(占12.5%)为高自发荧光,1眼(占12.5%)为正常自发荧光;慢性组的11例眼中,8眼(占72.7%)显示为低自发荧光,3眼(占27.3%)显示为高自发荧光。SD-OCT图像显示视网膜浆液性脱离区的FAF表现为:急性组的8眼中,5眼(占62.5%)为低自发荧光,3眼(占37.5%)为高自发荧光;慢性组的11例眼中,8眼(占72.7%)呈现高自发荧光,3眼(占27.3%)为低自发荧光。结论:急慢性CSC患者FFA图像所显示的渗漏点部位以及SD-OCT图像上显示的浆液性脱离部位的视网膜区域呈现出不同的FAF表现。FAF检查可为临床上评估CSC患者黄斑区视网膜色素上皮(RPE)细胞的功能、选择治疗方法及判断预后提供依据。Objective:To investigate the characteristics of fundus autofluorescence (FAF) in patients with acute or chronic cen- tral serous chorioretinopathy (CSC), in whom leakage points were shown by fundus fluorescein angiography (FFA) and loca lized detachment in macular region is revealed by spectral domain optical coherence tomography (SD-OCT). Methods: A total of 19 CSC patients (19 eyes) with leakage points shown by FFA were enrolled, of whom 8 patients(8 eyes) with duration less than 6 months were allocated to acute CSC group and 11 patients(11 eyes) with duration more than 6 months were allocated to chronic CSC group. The characteristics of fundus autofluorescence (FAF) in 19 CSC patients were observed by spectral-domain optical coherence tomography (SD-OCT) combined with indocyanine green angiography (ICGA). Results: In the retinal area with leakage points revealed by FFA, the result of FAF varied. Among the 8 eyes of acute CSC group, hypoautofluoresecence was found in 6 eyes(75. 0%), hyperautofluorescence was found in 1 eye (12. 5 %), and isoautofluorecence was found in 1 eye ( 12. 5% ). Among the 1 1 patients of chronic CSC group, hypoautofluoresence was found in 8 eyes(72. 7% ), and hyperautoflu- orescence was found in 3 eyes (27. 3%). In the retinal area with serous detachment revealed by SD-OCT, the result of FAF varied. Among the 8 eyes of acute CSC group, hypoautofluoresence was found in 5 eyes(62. 5 %), and hyperautofluorescence was found in 3 eyes(37. 5 %). Among the 1 1 eyes of chronic CSC group, hyporautofluorescence was found in 3 eyes(27. 3%) and hyperautofluorescence was found in 8 eyes(72.7%). Conclusions: In the retinal area with leakage points revealed by FFA or serous detachment revealed by SIYOCT, the characteristics of FAF of acute or chronic CSC patients varied.
