循环肿瘤细胞与膀胱癌临床病理特征的相关性研究

摘　　要：目的探讨循环肿瘤细胞(CTC)与膀胱癌患者临床病理特征之间的相关性。方法采集78例膀胱癌患者(膀胱癌组)和10例同期非肿瘤患者(对照组)外周静脉血,运用流式细胞术检测CD45表达阴性且CK8/18/19表达阳性的CTC,分析CTC与膀胱癌患者临床病理特征的相关性。结果 78例膀胱癌CTC阳性率为34.6%(27/78),对照组均未检出CTC,两组CTC检出率比较,差异有统计学意义(P<0.05);膀胱癌CTC阳性率与肿瘤大小、数目、T分期以及病理分级有关(P<0.05);多因素Logistic回归分析显示,CTC阳性率与肿瘤大小、T分期和病理分级有关(P<0.05)。CTC阳性率肿瘤直径≥3cm者为51.52%(17/33),<3cm者为22.22%(10/45);肌层浸润者为59.09%(13/22),非肌层浸润者为25%(14/56);病理高分级者为52%(13/25),低分级阳性率为26.42%(14/53)。结论 CTC与膀胱癌患者肿瘤大小、浸润深度和病理分级具有相关性。Objective To investigate the association between circulating tumor cells （CTCs） and clinicopathologic charac‐teristics of bladder cancer .Methods The peripheral venous blood samples were collected from 78 patients with bladder canc‐er and 10 patients with non‐tumor diseases .CTCs in the samples were detected with flow cytometry with anti‐CD45 antibody and anti‐CK8/18/19 antibody . The association between CTC and clinicopathologic characteristics of tumors was analyzed . Results The positive rate of CTCs in bladder cancer was 34 .6% （27/78） ,while CTCs were not detected in any non‐tumor patients .The positive rate of CTCs in peripheral blood was correlated with tumor size ,number of tumor ,T stage ,and patho‐logical grade .In multivariate analysis ,positive CTCs showed correlation with tumor size ,T stage ,and pathological grade （ P〈0 .05） .Conclusion The positive rate CTCs is significantly correlated with the diameter ,T stage ,and pathological grade of bladder tumor .
