胃肠道间质瘤的超声表现与病理危险度分级的对照研究

摘　　要：目的:总结胃肠道间质瘤的超声表现,评价超声检查评估胃肠道间质瘤病变危险度的临床应用价值。方法:搜集经术后病理证实为胃肠道间质瘤的33例患者进行回顾性分析,总结其术前超声图像表现,对病变的各种超声特征与病变危险度分级之间进行对照研究。结果:33例胃肠道间质瘤中,高度危险性7例(21.2%),中度危险性9例(27.3%),低度危险性15例(45.5%),极低度危险性2例(6.1%)。在超声图像上,胃肠道间质瘤的危险度在肿瘤的最大径线、生长方式、边界、内部伴无回声区和瘤体内血流信号方面的差异具有统计学意义(P<0.05),而在肿瘤内部回声程度方面的差异无统计学意义(P>0.05)。结论:胃肠道间质瘤的超声图像表现对病变危险度的评估具有重要的临床价值,为该病治疗方案的选择和随访提供重要参考信息。Objective：To estimate clinical value of ultrasound in assessing different risks of gastrointestinal stromal tumors（ GIST）by summarizing ultrasonic features of GIST. Methods：The ultrasonic data of 33 cases with GIST confirmed by pathology were retrospectively studied and compared with different risks of pathologic findings by an-alyzing the ultrasonic features of GIST. Results：There were 7（21. 2% ）high - risk,9（27. 3% ）intermediated -risk,15（45. 5% ）low - risk and 2（6. 1% ）very low - risk lesions in the 33 GIST cases. In ultrasonography,maxi-mum diameter,growth pattern,contour,inside with echoless area and blood flow signals within tumors of GIST between different risk groups had statistical differences（P 〈 0. 05）,while internal echo level had no statistical difference（P 〉0. 05）. Conclusion：The ultrasonic features of GIST have important clinical value in assessing the different risks and can provide important reference information for selection of treatment and follow - up visit of GIST.
