鼻咽癌患者治疗前后外周血CD4^+ CD25^+调节性T细胞和CD19^+ CD35^+细胞的变化

摘　　要：目的探讨鼻咽癌患者治疗前后外周血CD4+CD25+调节性T细胞(CD4+CD25+Tr)比例和CD19+CD35+细胞比例的变化及其与鼻咽癌复发或转移的关系。方法临床分期为Ⅱ期的54例鼻咽癌患者,随访3年后,有复发或转移8例,无复发转移46例。采用流式细胞术检测治疗前后患者外周血CD4+CD25+Tr比例和CD19+CD35+细胞比例。结果与治疗前比较,54例鼻咽癌患者治疗后CD4+CD25+Tr比例下降(P<0.01),CD19+CD35+细胞的比例升高(P<0.01)。8例有复发或转移的鼻咽癌患者,上述两指标在治疗后,与治疗前比较差异均无统计学意义(P>0.05);而46例无复发或转移的鼻咽癌患者,治疗后CD4+CD25+Tr比例下降,CD19+CD35+细胞的比例升高(P<0.01)。结论检测鼻咽癌患者治疗前后外周血CD4+CD25+Tr比例和CD19+CD35+细胞比例,可能有助于及早发现鼻咽癌复发或转移。Objective To explore the changes in proportions of CD4^＋CD25^＋regulatory T cells（CD4^＋CD25^＋Tr） and CD19^＋CD35^＋cells in peripheral blood before and after treatment and its relationship with recurrence or metastasis in patients with nasopharyngeal carcinoma （NPC）.Methods Fifty-four patients with stageⅡNPC were followed up for three years,among them,8 cases were recurrence or metastasis and 46 cases were not.Flow cytometry was used to detect the proportions of both CD4^＋CD25^＋Tr and CD19^＋CD35^＋cells in peripheral blood before and after treatment.Results Among the 54 NPC patients,the proportion of CD4^＋CD25^＋Tr decreased significantly after treatment compared with that before treatment（P〈0.01）,and the proportion of CD19^＋CD35^＋cells increased significantly after treatment（P〈0.01）.The changes in two indexes mentioned above showed no significant difference before and after treatment in 8 NPC patients with recurrence or metastasis（P〉0.05）. Among the 46 NPC patients without recurrence or metastasis,the proportion of CD4^＋CD25^＋Tr decreased（P〈0.01） while the proportion of CD19^＋CD35^＋cells increased（P〈0.01）after treatment.Conclusion Detecting the changes in the proportions of both CD4^＋CD25^＋Tr and CD19 ＋CD35 ＋cells in peripheral blood before and after treatment may contribute to the early determination of recurrence or metastasis of nasopharyngeal carcinoma.
