摘 要:目的:探讨负压技术在正颌手术术后感染治疗中的应用。方法:对31例正颌手术术后感染治疗中负压引流、开放引流以及抗生素使用效应进行比较。结果:负压引流联合全身抗生素应用的17例下颌双侧感染患者,4~6d后体温及血清学指标恢复正常,7~10d创口愈合,平均(8.9±0.6)d;局部开放引流联合全身抗生素应用的8例下颌单侧感染患者,4~7d后体温及血清学指标恢复正常,6例创口13~18d愈合,平均(15.1±1.1)d,2例3周后出现复发,再次清创并行负压处理后21d创口愈合;单独运用负压引流的4例下颌单侧及2例双侧患者,7~10d后血清学指标恢复术前状态,11~14d创口愈合,平均(12.4±0.9)d。相对于开放引流在下颌正颌手术术后感染的运用,负压封闭引流缩短了治疗时间(P〈0.05)。结论:清创引流是正颌外科术后感染治疗的重要手段,局部负压技术缩短正颌术后感染治疗时间,具有与全身抗生素使用的协同治疗效应。Objective To explore vacuum sealing drainage technology applied in the treatment of postoperative orthognathic surgery infection. Methods Comparison of the effect of vacuum sealing drainage,open drainage and systemic antibiotic on the treatment of postoperative infection by assessing temperature and serological indexes and wound healing in 31 orthognathic surgery cases. Results Patients of bilateral mandibular infection with systemic antibiotics application and vacuum sealing drainage of 17 cases,temperature and serological indexes restored to normal after treatment of 4-6 days,wound healing during 7-10 days,an average of(8.9 ± 0.6)days.Patients of unilateral mandibular infection with systemic antibiotics and open drainage of 8 cases,6 cases of temperature and serological indexes restored to normal after treatment of 4-7 days,wound healing during 13-18 days, an average of 15.1 ± 1.1 days,and 2 cases of recurrence appeared after 3 weeks.2 cases of patients get cured after treatment of 21 days with debridement and vacuum sealing drainage treatment.Patients of both 4 cases of unilateral and 2 cases of bilateral mandibular infection with only vacuum sealing drainage,serological indexes restored to normal after treatment of 7-10 days,wound healing during 11-14 days, an average of(12.4±0.9)days. Conclusion Debridement and drainage is an important means for the treatment of postoperative orthognathic surgical infection. Application of vacuum sealing drainage technology shorten the duration of treatment of orthognathic postoperative infections and appears a coordinated therapeutic effection with systemic antibiotics.
