摘　　要：目的 探讨硫酸镁预防动脉瘤性蛛网膜下腔出血（aSAH）患者脑血管痉挛的有效性.方法 系统检索Pubmed、Medline （OVID）、EMBase、中国期刊全文数据库（CNKI）以及万方医学网等数据库,收集硫酸镁预防性应用于动脉瘤性蛛网膜下腔出血患者的随机对照试验（RCT）研究.对符合本研究纳入标准的和既往Meta分析文献的“参考文献”逐条核对,对文献查漏排重.采用Jadad评分量表标准评价文献质量,应用RevMan 5.2软件进行Meta分析.结果 共有9项研究符合纳入标准,2314例患者接受了不同指标的试验观察.与对照组比较,硫酸镁组能够降低症状性脑血管痉挛（CVS）发生率,两组间差异有统计学意义[相对危险度（RR）=0.81,95％可信区间（CI）：0.66 ～0.99,P＜0.05];硫酸镁组与对照组aSAH后的死亡率差异无统计学意义（RR =0.97,95％ CI：0.78 ～1.19,P＞0.05）.结论 与对照组比较,硫酸镁组对aSAH后症状性脑血管痉挛或者迟发性脑缺血的发生有改善作用,但死亡率两组比较差异无统计学意义.Objective To evaluate the effectiveness of the magnesium sulfate in patients of aneurysm subarachnoid hemorrhage （aSAH） with cerebral vasospasm.Methods Retrieve databases include Pubmed,Medline （OVID）,EMBase and Wanfangdata,collected the randomized controlled trials （RCT） researches of the magnesium sulfate in patients of aSAH.Check the literatures which comply with a standard and analyzed by Meta,investigated repetitive literatures.Evaluated the quality of literatures followed the standard by Jadad Scale,analyzed by software RevMan 5.2.Results Tatal 9 researches were according to the standard,2314 patients accepted test.Contrast to controls,intervention groups reduced incidence of CVS significantly [relative risk（RR） =0.81,95％ confidence interval （CI）：0.66-0.99,P＜ 0.05];No statistical significance between controls and intervention groups （RR =0.97,95％ CI：0.78-1.19,P ＞ 0.05）.Conclusion Compared with control group,the magnesium sulfate group could reduce the incidence of delayed cerebral ischemia caused by symptomatic cerebral vasospasm after aSAH,but no significant statistical differences in mortality.
