摘 要:目的 探讨富含半乳糖凝集素-1(Galectin-1)基因修饰的骨髓间充质干细胞(MSC)的脱细胞真皮基质(ADM)补片移植修复大鼠肛门外括约肌损伤的可行性.方法 取Wistar大鼠制成肛门括约肌损伤模型.两周后按对损伤处置的方式分为分成A、B、C、D4组.A组直接行断端修补;B组断端修补+覆盖ADM补片;C组断端修补+覆盖富含MSC-ADM补片;D组断端缝修补+覆盖富含Galectin-1基因修饰的MSC-ADM补片.术后6周,切取标本.苏木素-伊红(HE)和Masson染色观察组织形态学改变;共聚焦显微镜观察Galectin-1表达;直肠肛管测压评估肛门括约肌功能.结果 与A、B、C组比较,D组大鼠损伤肛门括约肌修复处绿色荧光蛋白表达丰富,细胞形态呈肌细胞样;胶原纤维沉积显著减少,血管新生增加及肌纤维增多;肛门外括约肌收缩压力检测,A组收缩压力最大值(5.8±1.2) mmHg(l mmHg=0.133 kPa),B组(10.7±2.1) mmHg;C组(11.3±2.6) mmHg;D组(14.1±1.9)mmHg;与其他3组比较,D组显著高于其他各组,差异有统计学意义(P<0.05).结论 富含Galectin-1基因修饰MSC的ADM补片对大鼠损伤的肛门外括约肌具有良好的修复作用.Objective To investigate the feasibility of bone marrow mesenchymal stem cells (MSCs) overexpressing galectin-1 with the acellular dermal matrix (ADM) biological patch in treating external anal sphincter (EAS) injury of rat models.Methods Forty Wistar rats with external sphincter injury were randomly divided into four groups.Group A was given end to end sphincteric repair directly.Group B was subjected to end to end repair and then covered by ADM patch.Group C received end to end repair then was covered by ADM which was previously seeded with MSCs.Group D underwent end to end repair and then covered by ADM which was previously seeded with MSCs infected with pAdxsi-GFP GFP-galectin.After six weeks,all rats were sacrificed and the whole ring specimens including anal canal and lower rectum were removed.The hematoxylin eosin stain and Masson trichrome stain were performed to observe the change of histomorphology.EAS manometry was done immediately before sphincterotomy and 6 weeks after transplantation in each group.Results As compared with groups A,B and C,group D showed better compatibility of MSCs and ADM,lower collagen deposition,dramatically increase in dilated blood vessel,and significant improvement in EAS contraction curves [(14.1 ± 1.9) vs.(5.8 ± 1.2),(10.7 ± 2.1) and (11.3 ± 2.6) mmHg;P < 0.05].Conclusion Transplantation of ADM biological patch rich in MSCs overexpressing galectin-1 resulted in muscle regeneration,lower collagen deposition and recovery of the injured EAS.
