摘 要:目的:比较不同手术方式治疗翼状胬肉的临床疗效。方法:对医院翼状胬肉患者120例(137眼)病例资料进行回顾性分析。根据手术方式分为以下3组:A组(胬肉单纯切除联合丝裂霉素C)38例(43眼);B组(单纯切除联合羊膜移植)34例(38眼);C组(单纯切除联合带角膜缘干细胞移植)48例(56眼)。比较3组间年龄、胬肉长度、入选前手术次数,以及术后复发率的差异。结果:A、B、C组患者平均年龄分别为(52.9±1.9)、(54.1±2.1)、(53.6±2.3)岁;翼状胬肉长入角膜长度分别为(2.56±0.30)、(2.75±0.32)、(2.65±0.29)mm;入选前手术次数分别为(1.32±0.76)、(1.20±0.62)、(1.15±0.71)次。3组间年龄、胬肉长度、入选前手术次数比较差异均无统计学意义(P>0.05);3组患者术后复发率分别为11.63%、10.53%、5.36%,C组复发率显著低于A组、B组(P<0.05)。结论:3种手术方式均能有效治疗翼状胬肉,以翼状胬肉切除联合带角膜缘干细胞移植治疗效果最佳。Objective:To observe and compare the therapeutic effects of pterygium excision combined with mitomycin C,amniotic membrane transplantation or limbal stem cell transplantation in the treatment of pterygium.Methods:120cases(137eyes)of pterygium patients had been retrospectively analyzed.According to the operation mode,they had been divided into 3groups:pterygium excision combined with mitomycin C in group A of 43eyes;pterygium excision combined with amniotic membrane transplantation in group B of 38eyes;pterygium excision combined with limbal stem cell transplantation in group C of 56 eyes.The patient age,the length of pterygium,the ophthalmologic surgery time before selection and the recurrence rate after operation had been compared among these three groups.Results:The average age in three groups was(52.9±1.9),(54.1±2.1),(53.6±2.3)years,respectively.The length of pterygium was(2.56±0.30),(2.75±0.32),(2.65±0.29)mm respectively.The ophthalmologic surgery time before selection was(1.32±0.76),(1.20±0.62),(1.15±0.71)respectively.There was no significantly difference in age,length of pterygium,ophthalmologic surgery time among these three groups(P>0.05).The recurrence rate was 11.63%,10.53%,5.36% respectively in three groups,and it was much smaller in Group C than Group A and Group B(P<0.05).Conclusion:Three kinds of operation are all effective to treat pterygium,but the operation of pterygium excision with limbal stem cell transplantation is the ideal one.
