超声引导下行PTCD术的导管相关性感染因素分析

摘　　要：目的对超声引导下采用单腔静脉导管经皮肝穿刺胆道引流(PTCD)导管相关性感染进行原因分析并制定有效的预防措施。方法回顾30例超声引导下使用单腔静脉导管行PTCD的患者,分析感染病例的具体病情,并且针对术前、术中、术后三个阶段制定预防措施。结果术后发生不同程度感染者共7例,感染率23.3%;经临床分析,确定与引流管密切相关者3例,占感染患者的42.9%;介入超声医师术前准备、术中无菌操作以及术后引流管护理等环节均为导管相关感染重要因素。结论超声引导单腔静脉导管行PTCD术具有感染风险;介入超声医师应该高度重视,减少导管感染发生,提高临床治疗效果。Objective To analysis the relativity between single lumen catheter percutaneous liver and gallbladder catheter drainage （ PTCD ） guided under ultrasonography with its related infections and make effective prevention measures.Methods 30 cases with PTCD guided under ultrasonography were analyzed for their specific cases and carried out precaution measure for preoperative ,intraoperative and postoperative three period .Results There were 7 infections cases of different level in the postoperative period ,infection rate was 23.3%.And 3 infections were linked with drainage tube,which accounting for 42.9%of the infected patients.Interventional ultrasound doctors preoperative preparation ,intraoperative sterile operation and postoperative drainage tube care were important factors for catheter re-lated infections .Conclusion Single lumen catheter line PTCD guided by ultrasound technique has the risk of infec-tion,and interventional ultrasound doctors should attach great importance to reducing catheter infection and improving the efficacy of clinical treatment .
