摘  要:目的:探讨甲状腺良恶性结节超声造影特征及定量参数与病理微血管密度(MVD)的相关性,为甲状腺良恶性结节的不同灌注特点提供病理依据。方法:回顾性分析122例甲状腺结节的超声造影表现,进行时间-强度曲线(TIC)分析。对术后标本行免疫组化CD31、CD34染色后计数MVD,对甲状腺结节超声造影定量参数与MVD计数进行相关性分析。结果:在超声造影增强达峰时,甲状腺癌多表现为不均匀低增强,结节性甲状腺肿多表现为等增强,腺瘤表现为高增强。甲状腺癌组IMAX、MVD均低于结节性甲状腺肿及腺瘤组,差异均具有统计学意义(P<0.05)。甲状腺结节超声造影定量参数中IMAX与MVD呈显著正相关(rCD31=0.814,PCD31<0.01;rCD34=0.803,PCD34<0.01)。结论 :低增强是诊断甲状腺癌重要的增强模式,甲状腺癌呈乏血供状态。Objective: To investigate the performance of thyroid nodules with contrast-enhanced ultrasound (CEUS) and the correlation between CEUS quantitative parameters and microvessel density(MVD), to provide pathologic basis for different perfu- sion characteristics of benign and malignant thyroid nodules. Methods: The CEUS features of 122 patients with thyroid nod- ules were retrospectively analyzed. The time-intensity curves (TIC) were analyzed by TomTec software. Immunohistochemical staining were performed to evaluate the MVD in the surgical specimens, and the correlation of quantitative parameters with MVD were assessed. Results: When the ultrasound contrast enhancement to the peak, the thyroid carcinoma showed homoge- neous hypo-enhancement, nodular goiter showed iso-enhancement, thyroid adenoma showed hyper-enhancement. IMAX and MVD of thyroid carcinoma were lower than nodular goiter and thyroid adenoma, and the differences were statistically signifi- cant(P〈0.05). A positive correlation existed between the IMAX and MVD (rCD31=0.814,PCD31〈0.01; rCD34=0.803, PcD34〈0.01). Con- clusions: Hypo-enhancement was the important CEUS enhancement patterns to diagnose thyroid carcinoma, thyroid carcinoma was lack of blood supply.
