甲状旁腺素和血钙水平表达与甲状腺微小乳头癌切除范围的关系分析

摘　　要：目的研究甲状旁腺素(PTH)和血钙检测结果与甲状腺微小乳头癌切除范围的关系,以期为临床诊治提供参考。方法选择我院收治的甲状腺微小乳头癌患者180例,根据患者不同的手术方式分为A、B、C、D、E组,对患者术前1天、术后第1、3、5天的血钙水平以及甲状旁腺素(PTH)水平进行检测记录,并经行统计分析。结果术后各组内比较血清PTH和血钙水平,在第1、3、5天均比术前的血清PTH和血钙低(P<0.05);患者术后第1天的低血钙、低PTH发生率在A组和E组比较高,其他三组中低血钙发生率最高的是B组,低PTH的是D组。比较A、B、E组的低血钙发生率,相互之间差异有统计学意义(AE:χ2=3.864,P=0.029;AD:χ2=5.223,P=0.021;DE:χ2=6.031,P=0.007);比较A、D、E组的低PTH发生率,三者相互之间差异有统计学意义(AE:χ2=5.516,P=0.013;AD:χ2=8.248,P=0.001;DE:χ2=4.843,P=0.024)。结论甲状腺切除手术的各种术式对于甲状旁腺及其均有一定程度的损伤,无论选择何种术式均应注意保护甲状旁腺。甲状腺近全切除术和患侧淋巴结清扫术的低钙血症和低PTH的发生率均较小,可以在临床工作中合理选择应用。Objective To explore the relationship between the levels of parathyroid hormone and serum calcium with resection of thyroid microcarcinoma. Methods 180 cases of patients with thyroid microcarcinoma were divided them into A, B, C, D, E group depending on the surgical approach. The serum levels of calcium and parathyroid hormone （PTH） in patients at one day before surgery, the 1,3 and 5 day after surgery were detected. Results The serum levels of PTH and serum calcium in patients after surgery were lower than that of before surgery（P 〈0.05）. The incidence of hypocalcemia and low PTH in the group A and group E at first day of post-operative were relatively higher. In the other three groups, the highest incidence of hypocalcemia was the group B, the highest incidence of low PTH was Group D. There was significantly difference on the incidence of hypocalcemia between group A, B and E （ P 〈 0.05 ）. The incidence of low PTH in group A, D and E had significant difference （ P 〈 0.05 ）. Conclusion Variety surgical procedures of thyroidectomy surgery have a certain degree of damage for parathyroid. Total thyroid resection and ipsilateral lymph node dissection could result a lower hypocalcemia and low incidence of PTH work. , and could be reasonably chosen to apply in clinical
