血清κ/λ轻链比值在多发性骨髓瘤合并肾功能不全与肾功能不全患者中的鉴别诊断价值

摘　　要：目的比较血清轻链κ(kappa)、λ(lambda)浓度的测定及κ/λ比值在多发性骨髓瘤(multiple myeloma,MM)和肾功能不全患者中的不同,探讨其诊断价值。方法采用速率散射免疫比浊法测定血清轻链κ、λ,并计算κ/λ比值。结果各组血清κ/λ比值分别为:肾功能不全患组(1.99±0.72),κ型MM伴肾功能不全组(9.74±7.39),λ型MM伴肾功能不全组(0.27±0.24),肾功能不全组与MM伴肾功能不全组比较,差异有统计学意义(P<0.05)。结论对于肾功能不全的老年患者,血清轻链浓度的测量及κ/λ比值计算是一个方便、快捷、有效的方法,有助于鉴别诊断,更大程度的降低MM患者的误诊、漏诊率。Objective To investigate the value of serum light chains κ（ kappa）, λ（lambda） and κ/λ ratio in differential diagnosis of multiple myeloma （MM）with renal insufficiency and renal insufficiency patients. Methods The serum light chains in multiple myeloma with renal insufficiency and renal insufficiency patients were measured by immune rate nephelometry, and κ/λ ratios were calculated. Results The κ/λ ratio in renal insufficiency patients group was 1.99 ±0.72, in MM- κ with renal insufficiency patients group was 9.74 ± 7.39, and in MM- λ with renal insufficiency patients group was 0.27 ± 0.24. There was significantly difference between the renal insufficiency group and MM with renal insufficiency group （ P 〈 0.05 ）. Conclusion In elderly patients with renal insufficiency, the measurement of serum light chains and calculation of κ/λ ratio could be a convenient, fast, effective method for differential diagnosis of multiple myeloma with renal insufficiency and renal insufficiency patients.
