摘 要:为检测朊蛋白在头颈部正常粘膜、头颈部鳞状细胞癌(head and neck squamous cell carcinomas,HNSCC)以及其各亚组之间的表达及其临床意义。采用免疫组化SP法检测HNSCC(212例)和正常黏膜(84例)中Pr P的表达情况,并分析其与年龄、组织病理分期、临床分期等临床参数的相关性。结果显示,Pr P在正常黏膜、HNSCC中的阳性表达率为10.71%(9/84)、66.51%(141/212),差异有统计学意义(P<0.05),Pr P在高分化、中分化、低分化中的阳性率分别为28.36%(19/67)、68.12%(47/69)、98.68%(75/76),即阳性率随分化程度下降而升高(P<0.05)。Pr P分别与病理分期、TNM分期、淋巴结转移、发生部位表达差异有统计学意义(P<0.05),而不同年龄组表达差异无统计学意义(P>0.05)。在TNM分期中,Ⅰ、Ⅱ期Pr P表达强度随分化程度下降而明显升高(P<0.05),但与Ⅲ、Ⅳ期之间的差异无统计学意义(P>0.05);而在头颈部正常粘膜,I+Ⅱ(早期)和Ⅲ+Ⅳ期(晚期)的总体表达差异中有统计学意义(P<0.05)。由此可知,Pr P在HNSCC中的高表达与HNSCC的发生、病理分期、及临床TNM分期密切相关,可能为HNSCC的早期诊断和预后判断提供依据。To examine Prion protein(PrP) expression and its clinical significance in normal mucosa, head and neck squamous cell carcinoma (HNSCC)and its subgroups.Expression of PrP in HNSCC and mucosa specimen was detected by immunohistochemistry. The association between the expression and age,TNM clinical stages,pathological grades was evaluated.The positive expression rate of PrP in normal and HNSCC tissues was 10.71%(9/84) and 66.51%(141/212),respectively.There was a significant difference between the expression of PrP in pathological grades,TNM clinical stages,lymph node metastasis and different parts(P〈0.05).The positive expression rate of PrP in well differentiation,moderately differentiation and low differentiation were 28.36%(19/67), 68.12% (47/69) and 98.68% (75/76).The positive expression rate was increased with the declining of pathological differentiation (P〈0.05).There was no significant difference between PrP expression and age.PrP expression of stage I and II was up-regnlated with the decreased differentiation (P〈0.05).There was no significant difference in PrP expression between stage III and IV(P〉0.05). Among normal mucosa, expression of PrP in the stage Ⅰ+Ⅱ and Ⅲ+Ⅳ was a significant different(P〈0.05).The high expression of PrP in HNSCC and the progressive expression from normal mucosa to HNSCC is closely related to the carcinogenesis of HNSCC,pathologic stage and clinical TNM stage.
