奥马哈系统在居家脑卒中患者健康相关行为访视中的应用

摘　　要：目的探讨奥马哈系统在居家脑卒中患者健康相关行为问题的访视应用效果,为社区慢性病患者居家访视的发展提供依据。方法应用奥马哈系统(Omaha System)构建健康相关行为问题的居家访视评估表,随机选择176例居家脑卒中进行3次访视,对最常见的问题进行干预。结果患者或家属药物治疗方案、营养、个人照顾和身体活动问题的认知、行为、状况评分(K-B-S评分)都随着访视次数的增加而提升,访视前后评分改变显著差异有统计学意义(P<0.01)。结论应用奥马哈系统居家访视,能提高居家脑卒中患者的健康相关行为问题的K-B-S评分,提高居家脑卒中患者健康相关行为的认知,改变居家脑卒中患者不良的健康相关行为,从而帮助居家脑卒中患者建立健康的行为模式。Objective To explore the application effect of Omaha System in the study of health-related behavior problems of the home stroke patients when interviewing them at their homes to provides the reference for the development of home-interview of the chronic disease patients for the communities. Method Establishing the assessment system of the health related behavior problems with the Omaha System for home-interview. Randomly selecting 176 cases of the home stroke patients; Paying home-visits to them 3 times and conducting the intervention of the most common problems. Results The cognition, behavior, status score （K-B-S scores） of the patients or family members about the drug treatment solution, nutrition, personal care and physical activity were improved with the increase of numbers of visits score variation differences were significant and meaningful in statistics （all P 〈 0.01）. Conclusion The application of the Omaha System in the home-interview of the home stroke patients can raise their K-B-S scores of health-related behavior problems and their cognition of the health-related behavior and improve their health-related behavior so as to help them to establish the healthy behavior patterns.
