不同手术方式对子宫肌瘤患者卵巢功能的影响

摘　　要：目的探讨不同手术方式处理子宫肌瘤后对患者卵巢功能的影响。方法选择本院2009年1月至2012年2月收治的单发子宫肌瘤患者98例为研究对象,按手术方式不同将患者分为A、B、C三组,A组患者采取子宫全切术,B组患者采取次全子宫切除术,C组患者采取子宫肌瘤切除术,比较三组患者手术前后性激素水平变化。结果三组患者术前促卵泡生成素(FSH)、促黄体生成素(LH)及雌二醇(E2)水平比较差异均无显著性(P>0.05);术后3、6、12、24个月A、B组患者FSH和LH水平均较术前明显升高,E2水平较术前降低(P<0.05);C组患者术后FSH、LH及E2水平与术前比较差异均无显著性(P>0.05)。术后3、6、12、24个月A、B组患者FSH和LH水平均高于C组,E2水平低于C组(P<0.05)。结论子宫切除对卵巢功能具有明显影响,降低患者体内雌激素水平,子宫肌瘤患者选择子宫切除术时应严格掌握适应证,避免引起性激素分泌失衡。Objective To investigate the effect of different operation mode for hysteromyoma on ovarian function. Method 98 patients with hysteromyoma in our hospital from January 2009 to February 2012 were selected as objects of study. According to the different operation mode, patients were divided into group A, B and C. Group A received total hysterectomy, group B received subtotal hysterectomy, group C received myomectomy. The change of sex hormone levels in three groups of patients before and after surgery were compared. Result The level of follicle-stimulating hormone(FSH), luteotropic hormone(LH) and estradiol(E2) were identical in the three groups before surgery had no significant differences(P > 0.05). After 3, 6, 12, 24 months, FSH and LH levels in group A and B were significantly higher than preoperative, E2 levels were lower than preoperative(P < 0.05). The levels of E2, FSH, LH in group C after surgery had no significant differences compared with preoperative(P > 0.05). After 3, 6, 12, 24 months, the levels of FSH and LH in group A and B were higher than group C, E2 levels were lower than group C(P < 0.05). Conclusion Hysteromyoma has a notable effect on ovarian function in hysteromyoma, and decrease estradiol, that should be prudent for selecting hysteromyoma for patients with hysteromyoma, and provide unbalance of sex hormone.
