摘 要:目的评价多焦视网膜电图(mf-ERG)对视网膜震荡的诊断意义。方法应用Espion视觉电生理仪对临床确诊为单眼视网膜震荡62例伤眼及其对侧健眼进行mf-ERG检查。分别记录61个刺激单元的反映,分析比较5个离心度下视网膜的峰值振幅密度及隐含期。结果视网膜震荡伤眼5个离心度下mf-ERG峰值振幅密度在第一环与第二环伤眼较对侧健眼有明显下降,差异有统计学意义(P〈0.05),第三环至第五环振幅密度下降不明显,差异无统计学意义(P〉0.05);视网膜震荡伤眼对mf—ERG隐含期影响较小,在5个离心度中与对侧健眼差异均无统计学意义(P〉0.05)。结论视网膜震荡伤眼主要损伤在视网膜后极部的黄斑区,相对周边的视网膜损伤较小,多焦视网膜电图可以对视网膜震荡伤眼的视觉功能提供一种定位定量的检测方法。Objective To observe the diagnostic value of multifocal ERG for visual function on eyes of retinal concussion injuries. Methods Multifocal ERG examination was performed on 62 cases with monocular retinal concussion and their contralateral healthy eye by using Espion visual electrophysiology instrument. The reflecting of 61 stimulation unit was recorded and the amplitude density at 5 degrees of centrifugation and the implicit period was analysed and compared. Results Compared with the eontralateral healthy eye the mf-ERG peak amplitude at 5 degrees of centrifugation decreased significantly in the first ring and the second ring on the injury eyes and the difference was statistically significant ( P 〈 0.05 ). In the third to fifth ring the amplitude density decrease was not obvious, and the difference was not statistically significant ( P 〉 0.05 ). The retinal concussion had little impact on implicit period of mf-ERG and compared with contralateral healthy eye the difference was statistically significant ( P 〉 0.05 ). Conclusion The damage of concussion eye is mainly in the macular region and relatively less damage is found at surrounding retina. Muhifocal ERG can provide a quantitative test method to locate the visual function of retinal concussion.
