螺旋CT仿真内镜对小肠良恶性肿瘤诊断的价值

摘　　要：目的探讨螺旋CT仿真内镜对小肠良恶性肿瘤诊断的价值。方法选择玉田县医院2010年10月-2014年12月收治的疑似小肠肿瘤患者57例,均行CT仿真内镜（CTVE）检查。对CTVE检查结果与病理诊断结果进行Kappa检验,观察两种方法诊断结果的一致性。计算CTVE诊断原发性小肠肿瘤的准确度、灵敏度、特异度。结果病理检查结果：57例中恶性肿瘤27例,良性肿瘤30例。CTVE检查结果：57例中恶性肿瘤28例,良性肿瘤29例。两种方法诊断结果的一致性较好（Kappa≥0.70）。CTVE诊断原发性小肠恶性肿瘤的准确度、灵敏度、特异度分别为91.2%（52/57）、92.6%（25/27）、93.1%（27/29）。结论 CTVE技术操作相对简单,患者痛苦小,容易被接受。CTVE方法诊断原发性小肠肿瘤的准确度较高,并能反映病变全貌及其与周围组织的关系,能对病灶做出客观的评价,可为临床治疗方案的选择提供帮助。Objective To investigate the diagnostic value of spiral CT virtual endoscopy （CTVE） for the diagnosis of benign and malignant small intestinal tumors. Methods From October 2010 to December 2014, in Yutian County Hospital, 57 patients with suspected small intestinal tumor were selected as subjects. All patients were underwent CTVE examination. The CTVE results were compared with biopsy pathological diagnosis and the diagnosis consistency of the two methods were observed and analyzed by Kappa test. The accuracy, sensitivity, specificity of CTVE were observed in diagnosis of primary small intestinal tumors. Results Pathological results： among 57 cases, 27 cases were malignant tu- mors, 30 cases were benign tumors. CTVE results： among 57 cases, 28 cases were malignant tumors, 29 cases were benign tumors. Consistency test results showed that the two methods were good consistency （Kappa≥0.70）. The accuracy, sensitivity, specificity of CTVE examination were 91.2% （52/57）, 92.6% （25/27）, 93.1%（27/29） respectively. Conclusion CTVE technology has the advantages of simple operation, little pain, easy to be accepted by patients. In the diag- nosis of primary small intestinal tumors, CTVE method has higher accuracy and can make an objective assessment of the lesion that reflect the whole lesion and its relationship with the surrounding tissue, which provides help for clinical treatment in all cases.
