血液净化联合加贝酯治疗重症急性胰腺炎的临床疗效观察

摘　　要：目的 探讨血液净化联合加贝酯治疗重症急性胰腺炎的临床效果。方法 选取2011年11月~2014年11月本院收治的114例重症急性胰腺炎患者作为研究对象,随机分为观察组与对照组,各57例。两组均给予常规治疗及持续性血液净化治疗,在此基础上观察组给予加贝酯进行治疗。比较两组体内的细胞因子水平（TNF-α、IL-6及IL-8）、APACHE-Ⅱ评分、腹痛消失时间、血清淀粉酶恢复时间、住院时间、临床疗效及安全性。结果 观察组治疗后的TNF-α、IL-6、IL-8水平显著低于对照组治疗后,差异有统计学意义（P〈0.05）。观察组治疗后的APACHE-Ⅱ评分显著低于对照组治疗后,腹痛消失时间、血清淀粉酶恢复时间及住院时间显著短于对照组,差异有统计学意义（P〈0.05）。观察组的总有效率显著高于对照组,差异有统计学意义（P〈0.05）。两组均未出现明显的不良反应。结论 血液净化联合加贝酯治疗重症急性胰腺炎效果显著,值得临床推广应用。Objective To explore the clinical effect of combination of continuous blood purification and octreotide in the treatment of severe acute pancreatitis. Methods 114 cases of severe acute pancreatitis from November 2011 to November 2014 in our hospital were selected and randomly divided into the observation group and the control group,57 cases in each group.All patients were given conventional treatment and continuous blood purification therapy,on the ba- sis,the observation group was treated with octreotide.The cytokine level （TNF-α,IL-6 and IL-8）,APACHE- Ⅱ score,abdominal pain disappear time,serum amylase recovery time,hospital days,the clinical efficacy and safety in two groups was compared. Results The level of TNF-α,IL-6 and IL-8 of the observation group after treatment was lower than that of the control group after treatment,with significant difference （P〈0.05）.The APACHE- II score of the observation group was lower than that of the control group,abdominal pain disappear time,serum amylase recovery time and hospital days of the observation group was shorter than that of the control group,with significant difference（P〈0.05）.The total effective rate of the observation group was higher than that of the control group,with significant difference （P〈0.05）.There was no adverse reaction during treatment in two groups. Conclusion Continuous blood purification combined with octreotide in the treatment of severe acute panereatitis is effective and safe,it is worthy of clinical promotion and application.
