老年萎缩性胃炎患者血清同型半胱氨酸、胃蛋白酶原、叶酸及维生素B12水平检测结果分析

摘　　要：目的探讨血清同型半胱氨酸、蛋白酶原(PG)、叶酸及维生素B12在老年萎缩性胃炎中表达的特点。方法选择117例老年萎缩性胃炎患者,按照幽门螺杆菌检测阴、阳性结果,将患者分为A组(阳性)和B组(阴性),另外选择同期进行健康体检的人群共52例作为C组。检测比较三组研究对象叶酸、维生素B12、胃蛋白酶原、以及血清同型半胱氨酸水平。结果 A组患者的胃蛋白酶原I(PGI)水平、与胃蛋白酶原II(PGII)的比值(PGR)(PGI/PGII)显著低于B组和C组,差异具有统计学意义(P<0.05);三组研究对象PGII水平比较差异无统计学意义(P>0.05)。A组患者的血清同型半胱氨酸水平显著高于B组患者,B组患者的血清同型半胱氨酸水平显著高于C组健康人群,差异具有统计学意义(P<0.05);A组患者叶酸水平和维生素B12的水平显著低于B组患者,差异具有统计学意义(P<0.05),B组患者的叶酸及维生素B12水平与C组健康人群比较,差异无统计学意义(P>0.05)。结论老年萎缩性胃炎患者血清同型半胱氨酸水平较高,胃蛋白酶原水平较低,叶酸以及维生素B12水平较低,可能和幽门螺杆菌感染有关。Objective To analyze the characteristics of serum homocysteine, pepsinogen, folic acid and vitamin B12 in elderly patients with atrophic gastritis. Methods A total of 117 elderly patients with atrophic gastritis were divided into group A （positive group） and group B （ negative group） according to helicobacter pylori test. Another 52 cases who underwent physical examination during the same period were set as group C. Folic acid, vitamin B12, pepsinogen, and serum homocysteine were compared among the three groups. Results PGI level and PGI/PGII in the group A were significantly lower than those in the group B and group C, which had no significant difference （P 〈 0.05 ）. There was no significant difference in PGII among the three groups. Serum homocysteine in group A was obviously higher than that in the group B in which serum homocysteine was higher than that in the group C, which had significant differences（ P 〈 0.05 ）. Folic acid and vitamin B12 in group A was greatly lower than those in the group B, which had significant differences. However, compared with healthy people, Folic acid and vitamin B12 in group B had no significant difference. Conclusions Higher level of serum homocysteine and lower level of pepsinogen, folic acid, and vitamin B12 in elderly patients with atrophic gastritis might be relative to the infection of helicobacter pylori.
