消水散外敷联合胸腔灌注对恶性胸腔积液患者免疫功能的影响

摘　　要：目的:观察消水散外敷联合顺铂胸腔内灌注治疗恶性胸腔积液对免疫功能的影响。方法:将60例恶性胸腔积液患者随机分为2组,联合组30例采用消水散外敷联合胸腔内灌注顺铂,单纯组30例采用胸腔内灌注顺铂。比较2组临床疗效、免疫功能指标变化,对比不良反应发生情况。结果:在临床疗效方面,2组比较差异无统计学意义(P>0.05)。单纯组治疗前后CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+水平差异无统计学意义(P>0.05),但治疗后CD_3^+、CD_4^+、CD_4^+/CD_8^+水平均较治疗前下降,而CD_8^+水平较治疗前升高;联合组治疗前后CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+水平差异有统计学意义(P<0.05),且治疗后CD_3^+、CD_4^+、CD_4^+/CD_8^+水平均较治疗前升高,CD_8^+水平下降;2组患者治疗后CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+水平比较差异有统计学意义(P<0.05)。2组患者均有不良反应发生。结论:消水散外敷联合顺铂胸腔内灌注治疗恶性胸腔积液能够控制积液的生长,改善患者的免疫功能,减轻化疗药物不良反应,其机理可能是通过增强患者机体细胞免疫功能,抑制肿瘤细胞活性而发挥抗肿瘤作用。Objective ： To observe the effect of applying xiaoshui powder with cisplatin pleural perfusion on immune function of patients with malignant pleural effusion. Methods ： 60 cases of patients with malignant pleural effusion were randomly divided into two groups, 30 cases in each group. Combined group was received treatment of applying xiaoshui powder with cisplatin pleural perfusion, while simple group was treated with cisplatin pleural perfusion. The curative effects, the changes of immune functional parameter and adverse reactions between the two groups were compared. Results： There was no statistically difference between the two groups in curative effect（P 〉 0.05 ） ; the level of CD3 ^＋ , CD4^＋ , CD8 ^＋ , CD4 ^＋/CD8 ^＋ in simple group did not differed statistically before and after treatment （ P 〉 0.05 ）, while the level of CD3^＋ , CD4 ^＋ , CDsand CD4^＋/CD8 ^＋ had statistical differences in combined group（P 〈 0.05 ）, and which all rose after treatment, only the level of CD8 ^＋ fell ; after the treatment, the differences of the level of CD3 ^＋ , CD4 ^＋ , CD8^＋ and CD4 ^＋/CD8 ^＋ between the two groups were statistically sig- nificant（P 〈0.05 ） ; adverse reaction was occurred in the patients of both groups. Conclusion： Treating malignant pleural effusion with the treatment of applying xiaoshui powder with cisplatin pleural perfusion can control the growth of hydrops, improve the immune function of patients and relieve the adverse reaction of chemotherapeutics, the mechanism of antitumor effect is likely to be improving cellular immune function of patients and suppressing the activity of tumor cells.
