角膜基质内注射两性霉素B治疗真菌性角膜炎

摘　　要：目的探讨角膜基质内注射两性霉素B(AMB)治疗真菌性角膜炎(FK)的有效性和安全性。方法筛选确诊FK的病例70例(70眼),分为2组,采用AMB角膜基质内注射及滴眼联合治疗患者35例(35眼)为A组,单独应用AMB滴眼液滴眼治疗的患者35例(35眼)为B组;在联合抗真菌药物滴眼及全身治疗基础上,A组患眼病变区角膜基质内注射AMB注射液,观察疗效及不良反应,必要时重复注射。结果治疗第7天及第14天时,A组患者角膜溃疡面积较B组缩小,治疗第14天时A组角膜溃疡愈合、上皮完整的患者比例较B组高,A组的疗效较B组理想。未发现明显不良反应,22只无效眼联合手术治疗。结论角膜基质内注射AMB是治疗FK安全、有效的给药途径。Objective To investigate the clinical effect and safety of intrastromal amphotericin B injection in the treatment of fungal keratitis. Methods Seventy patients （70 eyes） with fungal keratitis were enrolled and divided into two groups. Thirty-five eyes received intrastromal amphotericin B injection combined with local and systemic anti-fungal drugs （group A）, while 35 eyes only received topical application （group B）. The therapeutic effect and side-effect were evaluated. Injection was repeated if necessary. Results The decrease in the area of keratomycosis in group A was significantly greater than that in group B at day 7 and day 14. The percentage of re-epithelialization in group A was significantly higher than that in group B at day 14. Group A had better therapeutic efficacy than group B. No obvious adverse reactions were observed. Twenty two eyes received surgical treatment. Conclusions Intrastromal amphotericin B injection is an effective and safe way to treat fuugal keratitis.
