应用P300评定中国老年期抑郁症患者认知功能的Meta分析

摘　　要：目的评价中国老年期抑郁症(LLD)患者的事件相关电位(ERP)P300潜伏期和波幅是否存在异常。方法制定文献的纳入标准、排除标准及检索策略,检索中国生物医学文献服务系统、万方数据库知识平台、维普期刊资源整合平台、中国科学文献服务系统、CNKI中国知网(总库)、Pub Med、Medline、Cochrane Library等。参考加强观察性流行病学研究报告的质量(STROBE)为文献质量评价标准。使用软件Rev Man 5.1对P300潜伏期和波幅进行Meta分析。结果共检索出109篇文献,对符合纳入和排除标准的8篇文献作Meta分析。结果显示,患者组与对照组之间P300潜伏期的标准化均数差(SMD)为0.90,95%CI为(0.45~1.36),患者组潜伏期较对照组延长(P<0.01);2组之间P300波幅SMD值为-1.23,95%CI为(-1.82^-0.64),患者组波幅低于对照组(P<0.01)。结论 P300能够反映LLD患者认知功能的损害,可以为LLD患者认知功能损害的早期评定提供一定的客观指标。Objective To investigate the latency and amplitude of event related potential（ ERP） using P300 in the Chinese patients with late-life depression（ LLD）. Methods The inclusion criteria,exclusion criteria and the search strategy were established,and literature retrieval was carried out from Chinese biomedical literature service system,Wanfang database knowledge platform,VIP Chinese journal resources integration platform,Chinese scientific literature service system,CNKI China（ pool）,Pub Med,Medline,Cochrane Library. Strengthening the reporting of observational studies in epidemiology（ STROBE） was used for quality assessment standard. Rev Man 5. 1 was used for Meta analysis of P300 latency and amplitude. Results Eight articles met the inclusion and exclusion criteria. The standardization mean difference（ SMD） of the latency of P300 between the patient group and control group was 0. 90,and 95% CI was（ 0. 45-1. 36）. The latent period of patient group was longer than that of the control group（ P 〈 0. 01）; SMD of the amplitude of P300 between the two groups was- 1. 23,and 95% CI was（- 1. 82-- 0. 64）. The amplitude of P300 of patient group was lower than that of the control group（ P 〈 0. 01）. Conclusions P300 can reflect the cognitive function in the patients with LLD,which could play an important role in early assessment for LLD with cognitive impairment.
