PASS直丝弓矫治器治疗上颌第一恒磨牙近中阻生的临床研究

摘　　要：目的研究PASS直丝弓矫治器治疗上颌第一恒磨牙近中阻生的临床效果。方法选择唐山市口腔医院替牙期上颌第一恒磨牙近中倾斜压迫上颌第二乳磨牙远中根并有不同程度吸收的患者25例，男15例，女10例；年龄7～11岁，平均9岁零2个月，替牙列，拔出上颌第二乳磨牙，让上颌第一恒磨牙自行萌出，呈近中倾斜状态。第一阶段PASS直丝弓矫治器结扎0．012Ni—Ti弓丝轻力直立上颌第一恒磨牙；第二阶段0．016澳丝保持第一阶段结果并Ni—Ti推簧推上颌第一恒磨牙向远中，恢复到正常位置。结果所有患者上颌第一恒磨牙牙冠均向远中移动（PTV—U6）4．7mm（P〈0．05），每侧可提供约5ram间隙供给上颌第二前磨牙萌出，上颌第一恒磨牙远中倾斜（SN—U6）8．2。（P〈0．05），上前牙唇侧倾斜（SN—U1）0．5。（P〈0．05），上下磨牙达到中性咬合关系或中性偏近中咬合关系，治疗时间4．0～6．0个月，平均5．1个月。结论PASS直丝弓矫治器是治疗上颌第一恒磨牙近中阻生的有效方法。Objecttive To investigate the effect of PASS straight wire appliance in the treatment of the maxillary first molar mesioangular impaction. Methods Twentyfive mixed dentition patiets （Male 15, Famale 10, average age 9 years and 2 months） were selected study subjects, whose distal roots of the maxillary second primary molar have different degrees of resorption by reason of the pressure of the maxillary first molar mesial tilting. After the removement of the maxillary second primary molar, the maxillary first molar erupted mesial tilting. First stage, we used PASS straight wire appliance with 0. 012 inch Ni - Ti wire to upright the first molar gently. Second stage, we used 0. 016 inch Australian wire with Ni - Ti helical spring to move the first molar distally to the right position. Results During the 4-6 months （5.1 months on average ） treatment period, the first molars of all patients moved distally （PTV-U6） 4.7 mm （P〈0.05）, witch can provide about 5mm for the eruption of the second premolar in each side. The first molars of all patients distally tilted （SN- U6） 8.2°（P〈0.05）, and upper anterior teeth lingual inclined （SN- U1） 0. 5°（P〈0. 05）. All patients obtained neutralizing interarch relationships or Neutral to slightly mesial occlusal. Conclusion PASS straight wire appliance is an effective appliance in treating the maxillary first molar mesioangular impaction.
