华蟾素胶囊联合化疗治疗晚期肺癌的临床研究

摘　　要：目的研究华蟾素胶囊对晚期肺癌患者凝血状态、治疗效果和生活质量的影响。方法 130例晚期肺癌患者随机分为治疗组(68例)和对照组(62例)。治疗组在标准化疗(吉西他滨/铂类、紫杉醇/铂类、培美曲塞/铂类或依托泊苷/铂类)的基础上口服华蟾素胶囊治疗,对照组仅给予标准化疗。21天为1个疗程。化疗≥2个疗程后,观察两组治疗后的凝血状态、肿瘤相关性血栓事件、治疗效果和生活质量。结果治疗后治疗组血浆纤维蛋白原、血小板、D-D二聚体水平均较对照组低,差异均有统计学意义(均P<0.05)。治疗组肿瘤相关性血栓事件的发生率均较对照组低,差异有统计学意义(P<0.05)。两组患者肿瘤标志物的变化差异无统计学意义(P>0.05)。治疗组与对照组疾病控制率差异有统计学意义(76.5%vs 64.5%,P<0.05),中位无进展生存时间和中位生存时间差异也均有统计学意义(15.4月vs 10.6月、21.8月vs 15.1月,均P<0.05)。治疗组的生活质量评分,尤其是疼痛的改善,优于对照组(P<0.05)。结论华蟾素胶囊联合化疗可改善晚期肺癌患者的血液高凝状态,提高近期疗效及改善患者的生活质量。Objective To evaluate the efficacy of cinobufacini capsule combined with chemotherapy in treatment of patients with advanced lung cancer. Methods One-hundred-and-thirty patients with advanced lung cancer were randomly divided into two groups： 68 patients in study group were given cinobufacini capsule on the basis of standard chemotherapy（ gemcitabine/platinum,paclitaxel/platinum,pemetrexed/platinum or etoposide/platinum）,62 in control group were given standard chemotherapy alone. The treatment was repeated every 21 d. Coagulation state,cancer-associated thrombosis,objective curative effect and long-term effect of two groups were compared after ≥2 cycles of therapy. The quality of life was assessed with EORTC QLQ-C30 scale before and after treatment. Results After treatment,the levels of plasma fibrinogen,blood platelet and D-D dimmer and the incidence rate of cancer-associated thrombosis in study group were lower than those in control group（ all P 〈0. 05）. There was no significant difference in tumor markers between two groups（ P 〉0. 05）.Study group was superior to control group in the disease control rate（ 76. 5% vs 64. 5%）,median progression-free survival（ 15. 4 months vs 10. 6 months） and median overall survival（ 21. 8 months vs 15. 1 months）（ all P 〈0. 05）. The quality-oflife scores in study group was higher than those in control group,especially in pain relief（ P 〈0. 05）. Conclusion Cinobufacini capsule combined with chemotherapy may enhance efficacy and improve blood hypercoagulation of patients with advanced lung cancer.
