子宫癌肉瘤106例临床分析

摘　　要：目的探讨子宫癌肉瘤的临床特点及预后影响因素。方法收集2000年1月至2012年1月广东省子宫内膜癌防治项目经手术病理确诊并有随访资料的106例子宫癌肉瘤病例。对其进行回顾性分析,探讨临床特点、组织学类型、手术病理分期、手术范围、术后辅助治疗的特点及其与预后的关系。结果子宫癌肉瘤的发病年龄为(55.8±0.9)岁。最常见首发症状为异常子宫出血,临床上多采用手术联合放化疗的治疗方案。总体生存时间92.4个月,中位生存时间78.8个月,3年、5年总体生存率分别为69.2%、55.3%。单因素分析显示:合并糖尿病、附件转移、肿瘤大小、手术-病理分期、术后辅助治疗与预后有关(P<0.05)。多因素分析显示:肿瘤大小、手术-病理分期是子宫癌肉瘤的独立预后因素(P<0.05)。结论子宫癌肉瘤是侵袭性肿瘤,早期诊断、综合治疗对提高子宫癌肉瘤的生存率有重要意义。Objective To analyze the clinical characteristics and evaluate the prognostic factors of the uterine carcinosarcoma （UCs）. Methods We retrospectively analyzed clinical pathologic data of 106 patients who were diagnosed with UCs from January 2000 to January 2012 in endometrial cancer prevention projects in Guangdong Province. We explored the clinical features, pathologic types, surgical pathological staging, scope of operation, postoperative adjuvant treatment and their relationship with prognosis. Results The average onset age was 55.8 ± 0.9 years old, while median onset age was 54, and it mostly occurred in patients from 50 to 59 years old. The 3-year and 5-year overall survival rate of UCs was 69.2% and 55.3%,the overall survival time was 92.4 months, and the median survival time was 78.8 months. In univariate analysis, combined diabetes, adnexa transfer, tumor size, surgical-pathological staging and postoperative adjuvant therapy were associated with prognosis （P 〈 0.05）. In multivariate analysis, only tumor size and surgical-pathological staging were the independent prognostic factors of UCs （P 〈 0.05）. Conclusion UCs is a high invasive uterine maligant tumor, and early diagnosis and comprehensive therapy have important significance to improve the survival rates.
