持续性躯体形式疼痛障碍的认知性电位P300研究

摘　　要：目的:探讨持续性躯体形式疼痛障碍(Persist Somatoform Pain Disorder,PSPD)认知性电位P300的特点,及其与症状严重程度的关系。方法:应用日本MEB-9200肌电图/诱发电位仪行P300检测,对60例PSPD患者(PSPD组)与60例健康成年人(normal control,NC组)P300潜伏期、波幅、靶刺激反应时及反应准确率比较;采用汉密尔顿抑郁(hamilton depression,HAMD)、汉密尔顿焦虑(hamilton anxiety,HAMA)、数字疼痛评定量表(numerical rating scale,NRS)评定症状严重程度,并探讨P300潜伏期与HAMD、HAMA和NRS评分的相关性。结果:与对照组相比,研究组Cz导联P300(P3a、P3b)潜伏期延迟、P300(P3b、N2-P3b)波幅降低、靶刺激反应时延迟与反应准确率降低(P<0.01),差异均具有统计学意义;P300潜伏期与HAMD、HAMA和NRS评分呈显著正相关(P<0.01)。结论:PSPD患者存在认知功能损害,焦虑、抑郁、疼痛影响PSPD患者的P300变化。Objective： To study the characteristic of event-related potentials P300 and relation between P300 and symptom severity of patients with persist somatoform pain disorder （PSPD）. Methods： With Japanese MEB-9200 Electromyography/Evoked potentiometer, P300 test was carried out in 60 patients with persist somatoform pain disorder （PSPD group） and 60 healthy adults （NC group）. The latency, amplitude, response times to target stimuli and response accuracy rates of P300 between two groups were compared and analyzed. The severity of symptom was evaluated with Hamilton depression （HAMD）, Hamilton anxiety （HAMA） and numerical rating scale （NRS）, and the relevance was explored between P300 latency and scores of HAMD, HAMA, and NRS. Results： Compared with those in control group, Cz lead P300 （ P3a, P3b ） latencies were delayed in PSPD group, P300 （ P3b, N2-P3b ） amplitudes were lower in PSPD group, response times to target stimuli and response accuracy rates were lower in PSPD group （P 〈 0.01）. All these differences were statistically significant. P300 latencies were significantly, positively related to the scores of HAMD, HAMA and NRS （P 〈 0.01）. Conclusion： Patients with persist somatoform pain disorder have cognitive impairments. Anxiety, depression, and pain affect P300 change of patients with PSPD.
