摘 要:目的探讨建立一种合适的疼痛管理模式,以提高疼痛护理质量,改善患者疼痛体验。方法以格林模式为理论框架,系统评估影响护士疼痛管理行为的各种因素,将其归纳为倾向因素、强化因素和促成因素,针对3种因素采取相应的措施,实现以护士为主导的疼痛管理。结果在应用格林模式实施以护士为主导的疼痛管理后,护士疼痛管理知识和态度问卷中各维度得分较前显著提高;疼痛评估记录规范率显著提高;患者术后12h、24h、36h、48h及72h疼痛评分显著低于模式建立前(P<0.05,P<0.01)。结论基于格林模式构建以护士为主导的疼痛管理模式,有利于强化护士疼痛知识和态度,促进护士疼痛管理行为的形成和维持,改善患者疼痛体验。Objective To establish a suitable pain management mode, so as to promote the quality of pain care and relieve patients' pain. Methods Based on Greenrs Precede Proceed Model, all the factors influencing nurses' management of pain were assessed and classified into the predisposing factors,reinforcing factors and enabling factors. Targeted measures were taken to realize the pain management dominated by nurses. Results After the implementation of the pain management dominated by nurses, the scores of nurses' knowledge and attitudes toward pain management were significantly higher than before (P〈0. 01 for all). The rate of standard recording of pain assessment improved significantly (P〈0.05,P〈0.01). Pain scores of patients 12 h, 24 h, 36 h, 48 h and 72 h after operation were significantly lower than before (P〈0.01 for all). Conclusion Application of pain management based on Green's Precede Proceed Model and dominated by nurses can strengthen nurses' knowledge about and attitudes towards pain, promote nurses' pain management behaviors and relieve patients' pain.
