产程体位干预配合视频宣教对枕后位产妇分娩的影响

摘　　要：目的探讨产程分阶段体位干预配合视频宣教对枕后位产妇分娩的影响。方法选取枕后位初产妇160例,采用随机数字表分为实验组和对照组各80例。实验组在产程不同阶段指导产妇使用针对性干预体位并配合视频宣教;对照组按照分娩期常规护理并根据产妇自己的意愿选择各种体位。观察两组分娩方式、产程时间、产后出血量、持续性枕后位及新生儿窒息发生率的差异。结果实验组较对照组第一、二产程时间缩短、产后出血少、持续性枕后位发生率低,自然分娩率(78.75%)高于对照组(53.75%),差异有统计学意义(P<0.05)。两组新生儿窒息发生率比较,差异无统计学意义(P>0.05)。结论产程体位干预配合视频宣教,有助于矫正胎儿枕后位,缩短产程,提高自然分娩率。Objective To explore the impact of the intervention of maternal posturing with video education at different stages on delivery in the occipitoposterior position. Methods A total of 160 cases of primiparas with occipitoposterior position were recruited and randomly divided into two groups（80 cases in each group）. The experimental group was given video education on delivery mechanism as well as the instruction of suitable positions at different stages of labor with occipitoposterior position. The control group was supported with routine nursing intervention and could take positions at their own will. The delivery mode,duration of labor,postpartum bleeding,incidence of persistent occipitopostefior position and the rate of neonatal asphyxia of two groups were observed. Results The first and second stage of labor in the experimental group were shorter than the control group（P〈0.05）. Compared with the con- trol group,the experimental group had a lower rate of persistent occipitoposterior position,lower rate of postpartum bleeding and higher rate of vaginal delivery（78.75% vs 53.75%）. There was no significant difference in the neonatal asphyxia rate of the two groups（P〉0.05）. Conclusions Intervention of maternal posturing with video education at different stage of labor helps relieve oeeipitoposterior position,shorten duration of labor and increase proportion of vaginal delivery.
