摘　　要：目的观察枳术丸联用西药对克罗恩病患者Th1细胞及CD8T细胞平衡状态的影响。方法选择收治的106例克罗恩病患者,随机分为枳术丸结合克罗恩病患者西药综合治疗组(观察组)及单纯克罗恩病患者西药治疗组(对照组),每组53例。治疗结束后,检测CD8T细胞群及T细胞亚群含量并评定两组患者的临床疗效。结果观察组有效率89.49%,优于对照组有效率72.57%(P<0.05)。治疗前,两组的CD8T细胞总数以及Tr、Th1的比率比较均无统计学差异(P>0.05);治疗后,观察组的CD8T细胞总数以及Th1的比率较对照组增加,而Tr的比率则较对照组降低(P<0.05)。结论枳术丸联用西药治疗克罗恩病患者有效性较好,其机制可能与其促进克罗恩病患者的Th1细胞分化,逐渐恢复Th1/Tr平衡有关。Objective To observe the influence in Th1 cells and CD8 T cell equilibrium after definite operation pill combination with western medicine for Crohn disease. Methods There are 106 patients with Crohn disease admitted in our department. All patients were randomly divided into Crohn disease western medicine comprehensive treatment in combination with chemotherapy pill group（ observation group） and pure western medicine treatment group（ control group） in patients with Crohn disease 53 cases in each group. After the treatment,the contents of CD8 T cells and T cells subgroup was detected,ratio are calculated. Th1 cells differentiation experiments,observation of r- interferon,IL- 1Β,IL-6,IL- 12 on the induction of Th1 cells,were used to assess the efficacy of the two groups. Results The effective rate in observation group was89. 49%. The effective rate was better than that of control group 72. 57%（ P〈0. 05）. Before the treatment,the total number of CD8 T cells and the proportion of Tr,Th1 in them in the two groups was no statistical difference（ P〈0. 05）. After treatment,the total number of observation group CD8 T cells and Th1 percentage in the control group increased,and the proportion of the Tr was relatively lower than control（ P〈0. 05）.Conclusion Definite operation pill combination of Crohn＇s disease western medicine has good effectiveness in the treatment of Crohn disease. Its mechanism may be and can be produced by the Β,IL- 6,IL- 1 cytokines,thus promotes the Th1 cells of Crohn disease differentiation,gradually restore balance on Th1 / Tr.
