摘 要:目的:观察胶原基纳米骨环用于颌骨缺损的成骨效果,探讨改良骨环技术用于临床扩增骨量的疗效及可行性。方法:10例牙列缺损拟接受种植修复的患者为研究对象,拟种植区因垂直骨质缺损而骨量不足,经排除手术禁忌后纳入研究并随机分为两组,对照组5例,采取标准GBR手术程序完成骨增量手术,加用1-2枚支抗钉维持成骨空间;实验组5例,采用标准骨环技术完成骨增量手术,将胶原基纳米骨环代替自体骨环,骨环以钢钉固定在骨缺损区。术前、术后即刻、术后6个月,分别对骨缺损区平行投照根尖片确定骨水平,观察骨水平变化,并行组内、组间数据配对t检验,评价骨增量效果。结果:两组骨缺损均得到了不同程度的恢复,两组间骨水平数据,组内术前、术后即刻比较有显著性差异,术前、术后6个月比较无显著性差异;组间比较均为显著性差异。结论:观察期内采用胶原基纳米骨环行改良骨环术扩增骨量不成功,值得推敲失败的原因。Objective: To observe the effect of bone augmentation use Nano-hydroxyapatite collogen block as a bone ring at insufficient alveolar area, and to discuss the feasibility on clinical using. Methods: 10 patients intended to have an implant-supported prosthodontics, with missed teeth and insufficient alveo- lar ridge, were taken part in this research program after contradication excluded, and divided into two groups randomly. Group A, 5 cases in control group accept standard GBR treatment for ridge augmen- tation, add 1-2 metal anchor for space support; Group B, 5 cases in experimental group accept standard bone ring technique, using hydroxyapatite collogen block as a autogenous bone graft,the block was fixed by metal anchor. All surgery area have been taken intraoral radiographs before surgery, immedi- ately after surgery and 6 months after surgery for regenerated bone level calculating, the data was treated by paired t test for evaluating the effect of bone augmentaion. Results: The effect of bone augmentation was obtained at some degree in both group, within each group, there is significant difference on bone level before surgery and immediately after surgery, but no significant difference between before surgery and 6 months after surgery. In different groups, there was no significant difference among before surgery, immediately after surgery and 6 months after surgery. Conclusions: Within the observation period, the effect on bone augmentation using hydroxyapatite collogen block as a bone ring at insufficient alveolar area can not reach clinical requirement, but worth to further research on this field.
