摘 要:目的:探讨应用组织工程技术中的支架技术和生长因子技术,在上颌后牙区重建种植体周围骨组织与软组织的临床成效。方法:选择上颌后牙区牙缺失伴骨量不足患者20例,随机分实验组对照组;实验组非细胞型组织工程技术重建上颌种植体周围组织。对照组使用常规重建种植体周围组织。精度修复后3、6、12月后以种植体动度仪测定法、曲面断层扫描法、CT多平面重建法、牙周指数测定法分别检测。结果:实验组种植体动度值、种植体周围边缘骨高度值、骨密度CT值、种植体生物学宽度值、种植体周围牙龈指数、种植体周围牙龈出血指数和种植体周围菌斑指数也明显优于对照组(P<0.01)。结论:非细胞型组织工程技术可有效重建上颌种植体周围组织,对提升种植修复的远期疗效具有重要的临床意义。Objective: To evaluate the clinic effects of cell free-tissue engineering technique in maxillary per- i-implant tissue reconstruction. Methods: A randomized controlled experiment was conducted. 20 adult patients with two or three teeth loss and bone defect in posterior maxilla were divided into two groups, experimented groups and controlled groups. Experimented groups were adopted cell free-tissue engi- neering technique to reconstruct maxillary peri-implant tissue, while controlled groups were adopted the regular one. Finally, implant-supported prosthetics were applied in both groups. Marginal bone levels and soft tissue alterations of peri-implants were measured through taking x-ray periapical films after 1,3 and 5 monthes of prosthetic loading. Results: Implant mobility index, height of peri-implant marginal bone, biological width of implants, peri-implant gingival index, peri-implant gingival bleeding index and peri-implant plaque index in experimented groups are much better than the controlled one (P〈0.01). Conclusion: Cell free-tissue engineering technique could reconstruct maxillary peri-implant tissue effectively and would contribute a lot in improving the long-term success rate of dental implant prosthetics.
