摘 要:目的比较卡马西平、拉莫三嗪、奥卡西平、托吡酯、丙戊酸钠对癫痫发作后状态的影响情况。方法采用非盲方法入选青岛市立医院登记并长期随访的癫痫患者180例,分别为服用卡马西平的癫痫患者30例,服用拉莫三嗪的癫痫患者30例,服用奥卡西平的癫痫患者30例,服用托吡酯的癫痫患者30例,服用丙戊酸钠的癫痫患者30例,新诊断、既往未服用抗癫痫药物的癫痫患者30例为空白对照组。用利物浦痫性发作严重程度量表2.0(LSSS2.0)对癫痫患者病情进行评价。抽取与癫痫发作后状态相关项目的评分进行比较。结果 6组患者癫痫发作后状态评分总体差异有统计学意义(F=3.105,P=0.010<0.05)。拉莫三嗪组、丙戊酸钠组与空白对照组比较差异有统计学意义(P<0.05)。卡马西平组、奥卡西平组、托吡酯组与空白对照组比较差异无统计学意义(P>0.05)。结论拉莫三嗪、丙戊酸钠单药治疗对改善癫痫发作后状态严重程度有良好效果,其它常用抗癫痫药物对癫痫发作后状态改善情况尚需进一步研究。Objective To compare the effect of carbamazepine ( CBZ ) , lamotrigine ( LTG ) , oxcarbazepine (OXC),topiramate(TPM) and sodium valproate(VPA) on the postictal state(PIS).Methods The study enrolled 180 cases of patients with epilepsy who were registered and long-term followed up in Qingdao Municipal Hospital ,such as 30 cases with CBZ ,30 cases with LTG ,30 cases with OXC ,30 cases with TPM ,30 cases with VPA and 30 cases who were newly diagnosed without antiepileptic druges ( AEDs) previously as the placebo group .The severity of seizure was evalua-ted by the Liverpool seizure severity scale 2.0(LSSS2.0).The scores of the items about PIS selected from LSSS2.0 were compared.Results There were statistically significant differences (F=3.105,P=0.010〈0.05) of the scores among the 6 groups.There were statistically significant differences between LTG group ,VPA group and the group without AEDs previously(P&lt;0.05).There were no statistically significant differences between the CBZ group ,OXC group,TPM group and the group without AEDs previously (P〈0.05).Conclusion LTG,VPA monotherapy can significantly improve the PIS.The effect of other popular AEDs on PIS remains further research .
