局部晚期鼻咽癌洛铂单药与PF方案同步放化疗临床观察

摘　　要：目的:比较洛铂单药用于局部晚期鼻咽癌(nasopharyngeal carcinoma,NPC)同步放化疗与顺铂(c i s p l a t i n,C D D P)、5-氟尿嘧啶(f l u o r o u r a c i l,5-F U)联合方案同步放化疗的疗效及毒副反应。方法:101例局部晚期NPC分为两组,49例接受CDDP+5-FU联合方案同步放化疗(PF组),52例接受洛铂单药同步放化疗(L组),比较近期疗效、5年生存率及放疗期间毒副反应。结果:PF、L两组患者放疗一月后客观有效率分别为9 5.9%、9 6.2%(P=0.8 8 9),5年生存率分别为5 3.1%、55.8%(P=0.921),毒副反应恶心呕吐和口腔黏膜反应PF组均较L组明显(P<0.05)。结论:洛铂单药用于局部晚期NPC同步放化疗与CDDP+5-FU联合方案同步放化疗的疗效近似,总生存率无统计学差异,洛铂单药组的恶心呕吐、口腔黏膜反应较轻。Object：To investigate the efficacy and adverse effects between lobaplatin alone and cisplatin （CDDP） combination with lfuorouracil （5-FU） plus concurrent radiotherapy for local-regionally advanced nasopharyngeal carcinoma. Methods：A total of 101 patients with local-regionally advanced nasopharyngeal carcinoma were divided into two groups, 49 patients received CDDP combination with 5-FU and 52 patients were given lobaplatin alone （group L）, respectively, plus concurrent radiotherapy. Compared of the two groups with short-term effects, 5 years survival rate and adverse effects during radiotherapy. Results：A month after radiotherapy, objective response rate was 95.9%for group PF and 96.2%for group L （P=0.889）, and 5 years survival rate was 53.1%vs. 55.8%（P=0.921）, respectively. Adverse effects, including nausea, vomiting and oral mucosa reaction, in group PF were more obvious than group L （P〈0.05）. Conclusion：hTe effcacy between lobaplatin alone and CDDP combination with 5-FU plus concurrent radiotherapy for local-regionally advanced nasopharyngeal carcinoma was similar. Total survival rate between two groups had no signiifcant difference. Nausea, vomiting and oral mucosa reaction in group L were signiifcantly lower than group PF.
