生物蛋白胶预防盆腔淋巴结清扫术后淋巴囊肿发生率的Meta分析

摘　　要：目的：评价生物蛋白胶在预防盆腔淋巴结清扫术后淋巴囊肿形成的疗效。方法：采用计算机检索Cochrane图书馆、Pub Med、Embase、ASCO、中国生物医学文献数据库、中国期刊全文数据库、万方数据库中从1993年1月至2014年12月31日关于生物胶预防盆腔淋巴结清扫术后淋巴囊肿形成疗效的相关文献。由两名评价者独立按Cochrane系统评价手册5.1版推荐的Cochrane风险偏倚评价工具评价纳入研究的质量,并采用Rev Man 5.3软件进行Meta分析,对纳入的RCT进行资料提取及数据合并,合并统计量为相对危险度（RR）。结果：纳入4项RCT共294例患者。Meta分析结果显示,生物蛋白胶能有效预防盆腔淋巴结清扫术后淋巴囊肿的形成（RR=0.53,95%CI=0.31-0.90）,差异有统计学意义。结论：目前证据表明,生物蛋白胶在预防淋巴囊肿形成上有明显优势,但由于纳入文献较少,且原始文献样本量小,故需对此结论持审慎态度,尚需进一步开展大规模、高质量的随机对照试验进行证实。Objective To evaluate the efficacy of fibrin for preventing lymphocele after pelvic lymphadenectomy in patients with gynecologic malignancies. Methods We searched the Cochrane Library,Pub Med,Embase,ASCO,CBM,CNKI and Wanfang datebases for studies published from January 1993 to December 2014 that evaluated the efficacy of the fibrin application for preventing lymphocele after pelvic lymphadenectomy in patients with gynecologic malignancies. Two reviewiers independently retrieved clinical controlled trials according to the inclusion and exclusion criteria,assessed the methodological quality of the included trials,and extracted data. Meta analysis was performed by Rev Man 5. 3 software. Results Four studies（ 294 patients） were ultimately included. The meta-analysis demonstrated that the application of fibrin seems to provide a useful additional treatment option for preventing lymphocle after pelvic lymphadenectomy. Those patients following the use of fibrin had significantly fewer lymphoceles（ RR = 0. 53,95% CI = 0. 31 - 0. 90）. Conclusion The application of fibrin seems to provide a useful additional treatment option for preventing lymphocle after pelvic lymphadenectomy. But the differences in trial design,especially the few number of included papers,limited the meaning of this conclusion. The further randomized control trials will be need to confirm the effect of fibrin.
