灾疫时期的公共卫生伦理探析

摘　　要：日益频发的重大灾疫引来了广泛的伦理关注。在应对灾疫中,既要解决医学问题,又要解决公共卫生伦理学问题。探讨其间评价公共卫生行动的效用、尊重、公正、互助和相称性等伦理学原则,分析卫生资源分配、应急救治职责、隔离检疫措施等伦理学问题。建议将公共卫生伦理学框架纳入政府应对灾疫的行动方案中,科学地制定公共卫生伦理学指南;准确把握灾疫现状,最优组合配置医疗资源,实现资源效用最大化;依法构建公共卫生安全应急体系,提升灾疫时期应对公共卫生安全的能力。The increasingly frequent outbreak of major disasters and epidemics has brought up extensive ethical concerns, including both medical issues and specific ethical issues of public health. In this article, the ethical principles of public health including effectiveness, respect, justice, mutual assistance and proportionality were discussed, etc. Meanwhile, the specific ethical issues about the distribution of public health resources, emergency care, isolation and quarantine were analyzed. Furthermore, the authors suggested that the public health ethics framework should be included in the government plan of responding to disasters, and a public health ethics guide should also be developed. At the same time, based on accurately evaluating the situation of disasters and epidemics, we should optimize the allocation of medical resources and maximize the utility of the resources. In addition, public health and safety emergency system needed to be established in order to enhance the ability of responding to disasters and epidemics.
