低位整束结扎胸导管法在预防乳糜胸中的作用研究

摘　　要：目的探讨食管癌手术中低位整束结扎胸导管法在预防术后乳糜胸中的作用。方法实施食管癌手术的178例患者,按是否在食管癌手术中低位整束结扎胸导管分成两组,在确保两组具有良好均衡性的前提下比较两组乳糜胸的发生率。结果 178例患者中,I期9例,IIa期50例,IIb期51例,III期57例,IV期9例,按肿瘤位置分为:颈及胸上段31例,胸中段150例,腹及胸下段61例;按肿瘤是否侵袭食管后壁(包括后壁、左后、右后、全周)与否分为:侵及食管后壁及未侵及食管后壁,在两组间的分布无统计学意义(P>0.05);乳糜胸发生率两组间有统计学差异。结论两组患者术后病理分期构成、肿瘤胸段位置情况、肿瘤是否侵袭食管后壁位置比差异无统计学意义(P>0.05),具有良好可比性,低位结扎胸导管能降低乳糜胸的发生率(P<0.05)。Objective To explore the role of low ligation thoracic duct method in prevention of breast postoperative chylothorax in esophageal cancer surgery. Methods 178 cases of esop cancer surgery patients, were devided into two groups according to whether undertake low ligate thoracic duct or not. Results Incidence of chylothorax between the two groups was statistically different. Conclusion That of the low ligation group was less than that of non-low ligation group. Low ligation of thoracic duct can reduce the incidence of chylothorax（P〈0.05）.
