乳晕旁切口治疗乳腺良性肿块的美容手术方式探讨

摘　　要：目的 探讨乳晕旁切口治疗乳腺多发良性肿块美容手术的临床效果.方法 选择2014年湖南省人民医院乳甲外科经乳晕旁切口切除乳腺良性肿块患者共406例,其中单纯乳晕旁切口美容术279例,乳晕旁切口联合麦默通旋切术42例,联合美兰染色标记56例,多种联合方式29例,随访其术后并发症、复发率及满意度等情况.结果 （1）单纯乳晕旁切口手术发生术后血肿的概率比乳晕旁切口联合两种或三种方式手术的概率明显较低（P＜0.01）.（2）单纯行乳晕旁手术切口的患者满意度比其他各组明显较高（P＜0.01）.（3）乳晕旁切口术后总并发症及总复发率均较低,并且乳房美容效果及患者满意度高.结论 乳腺旁切口美容手术效果理想,达到外科美学要求,值得临床推广.Objective To explore the cosmetic effect of periareolar incision treating the multiple benign breast lumps in clinic.Methods A total of 406 cases with benign breast lumps from Hunan Provincial People＇s Hospital treated by periareolar surgical resection were collected in 2014.Forty two of all cases were treated by periareolar surgical resection combined with mammotome resection,56 of all cases were treated by periareolar surgical resection combined with injecting methylene blue as a mark,and 29 of all cases were treated by combined therapy with three methods above.Follow-up and evaluation were performed about postoperative complications,recurrence rate and patients satisfaction scores using the statistical methods for data analysis.Results （1）The incidence of postoperative hematoma of whom treated only by periareolar surgical resection was significantly lower than that by periareolar surgical resection combined with two or three methods （P 〈 0.01）.（2）The patient satisfaction of whom treated only by periareolar surgical resection was significantly higher than other groups （P 〈0.01）.（3）The total postoperative complications and recurrence rate of the patients treated by periareolar surgical resection was low,the cosmetic effect of breast and patient satisfaction was high.Conclusions The cosmetic effect of periareolar incision which treats the multiple benign breast lumps is ideal.It can achieve the requirements of cosmetic surgery.
