摘  要:目的探讨淋巴瘤治疗前PET/CT18F-FDG摄取值在诊断分期及NHL亚型中的价值。方法回顾性分析2010年1月~2014年10月经病理学证实的59例治疗前行18 F-FDG PET/CT显像的患者,将患者不同的临床分期和病理类型进行分组,采用SPSS12.0软件评价临床各分期和各病理分型之间平均T/MB值之间相关性。结果临床分期中Ⅰ~Ⅱ期和Ⅲ~Ⅳ期患者的T/MB值为5.80±3.73和4.66±2.70(P=0.178),Ⅰ~Ⅱ期和Ⅲ~Ⅳ期之间差异无统计学意义。HL与NHLT/MB值为4.85±2.10和5.11±3.39(P=0.831),差异无统计学意义。NHL患者中,DLBCL与FL、SLL T/MB值为6.97±3.39、4.01±1.96和2.24±0.81。DLBCL与FL之间差异无统计学意义(P=0.058);DLBCL与SLL之间差异有统计学意义(P=0.03)。惰性NHL及侵袭性NHL T/MB值为3.20±1.86和6.47±4.03(P=0.002),差异有统计学意义。B细胞淋巴瘤与T细胞淋巴瘤T/MB值为5.51±3.60和3.34±2.22(P=0.111),差异无统计学意义。结论治疗前PET/CT18F-FDG摄取值在淋巴瘤临床分期中无相关性,但治疗前FDG代谢摄取值与肿瘤的恶性程度相关,在NHL各亚型分型中DLBCL与SLL之间,惰性NHL与侵袭性NHL之间差异有统计学意义。B细胞淋巴瘤与T细胞淋巴瘤之间差异无统计学意义。Objective To evaluate the role of FDG uptaking value with Pre-therapy 18F-FDG PET/CT imaging for diagnosis and staging in patients with lymphoma and NHL subtype. Methods Pre-therapy 18F-FDG PET/CT scans performed in 59 patients with lymphoma between January 2007 and August 2009 were assessed retrospectively, and the patients were divided into some subgroups according to clinical stages and pathological pattern. SUVmax was determined, and T/MB was calculated. The correlation of T/MB Between different subgroups with clinical stages or pathological pattern was evaluated by SPSS 12.0 software. Results In patients with clinical stagesⅠ-Ⅱ and Ⅲ-Ⅳ, T/MB was 5.80±3.73 and 4. 66±2.70 respectively, and there were no statistical significance of a difference in T/MB levels between stage Ⅰ-Ⅱ and Ⅲ-Ⅳ ( P =0. 178). T/MB of HL and NHL was 4. 85±2. 10 and 5.11±3.39 respectively, and there were no statistically significant of a difference ( P =0. 831). In NHL, T/MB of DLBCL and FIr, SLL was 6.97±3.39, 4.01±1.96 and 2.24±0.81, respectively, and there were no statistically significant differences between DLBCL and FL ( P =0. 058), but there were a statistically significant between DLBCL and SLL ( P =0.03). T/MB was 3.20±1.86 and 5.67±3.33 respectively in inertial NHL and invasive NHL, and there was a statistically significant difference ( P = 0. 002). T/MB was 5.51±3.60 and 3.34±2.22 respectively in B cell lymphoma and T cell lymphoma, and without statistically significant ( P=0. 111). Conclusion No correlation was discovered between FDG uptaking value of Pre-therapy 18 F-FDG PET/ CT imaging and clinical stages of lymphoma. And there were no statistically significant differences of FDG uptaking value between B cell lymphoma and T cell lymphoma. But there was a statistically significant difference between DLBCL and SLL, as well as inertial NHL and invasive NHL.
