摘 要:目的探讨触发式功能电刺激结合序列组合训练对脑卒中后足下垂患者步行能力的影响。方法选取40例脑卒中致足下垂的患者,分成实验组和对照组各20例。在常规药物治疗、康复治疗的基础上,对照组采用电刺激结合神经肌肉功能再训练肢体功能强化训练及步行训练;实验组采用特制触发式功能电刺激设备辅助步行治疗结合序列组合训练。比较两组治疗8周后Berg平衡测试、"起立-走"计时、10 m最大步行速度和2/6分钟步行能力。治疗每次60 min,每天2次,每周5天,共计8周。结果治疗8周后,实验组和对照组两组患者Berg平衡测试、"起立-走"计时、10 m最大步行速度、2/6分钟步行能力均较治疗前明显提高(t分别=-11.09、-9.25;5.19、5.95;-7.94、-6.25;-9.11、-7.69,P均<0.05);实验组2/6分钟步行能力明显优于对照组,差异有统计学意义(t=3.66,P<0.05)。结论触发式功能电刺激结合序列组合训练更有效改善卒中足下垂患者2/6分钟步行能力。Objective To explore the effect of gait triggered functional electrical stimulation combined with task-oriented circuit class training on walking ability in stroke patients with foot drop. Methods A total of 40 stroke patients with foot drop were included and randomly divided into the experimental group and control group with 20 cases in each. On the basis of conventional therapy, the control group patients were received the electrical stimulation combined with neuromuscular function training and walk-training, the experimental group was received gait triggered functional electrical stimulation combined with task-oriented circuit class training. The berg balance, timed up and go, walking speed (10m maximum walking speed, 10m MWS) and endurance (2/6 minutes walking test) were compared in two groups. Results After treat-ing for eight weeks, both two groups had the significantly improvement on berg balance , timed up and go, 10m MWS and 2/6 minutes walking test (t=-11.09,-9.25, 5.19, 5.95,-7.94,-6.25,-9.11,-7.69, P〈0.05). The walking endurance (2/6 minutes walking test) of experimental group was significantly greater than that of the control group(t=3.66, P〈0.05). Conclusion The trigger functional electrical stimulation combined with sequence combination training for stroke patients with foot drop can effectively improve the 2/6 minutes walking ability.
