摘 要:【目的】探讨实时灰阶超声造影(CEUS)在胰腺局灶性病变(SFLPs)诊断中的应用价值。【方法】选择SFLPs患者80例,分别进行常规超声和CEUS检查,并将其诊断结果与病理结果比较,分析良、恶性病变与正常胰腺实质始增时间、达峰时间及峰值强度等。【结果】通过手术或穿刺病理学诊断:57例为恶性病变,23例为良性病变;CEUS诊断:62例为恶性病变,18例为良性病变;常规超声诊断:42例为恶性病变,38例为良性病变。CEUS与常规超声诊断的灵敏度分剐为88.71%和73.81%(X2=3.884,P=0.049);特异度分别为88.89%和68.42%(X2=2.729,P=0.099);准确度分别为88.75%和55.00%(X2=23.921,P=0.000);阳性预测值分别为96.49%和54.39%(x2=27.269,P=0.000);阴性预测值分别为69.56%和52.17%(X2=1.460,P=0.227)。恶性病变CEUS开始增强的时间较胰腺实质晚(P〈0.05),开始减退时间较胰腺实质早(P〈0.05),渡过时间短于良性病变和胰腺实质(P〈0.05)。【结论】CEus作为一种安全、简便、无创的检查方法,在SFLPs中有较高的诊断价值,值得临床推广应用。[Objective]To study the diagnostic value of contrast-enhanced ultrasound on Solid Focal Lesions of pancreas (SFLP). [Methods]A total of 80 cases of SFLPs were selected from February 2013 to February 2015. All cases were undergone both routine ultrasound and contrast enhanced ultrasound (CEUS) examinations. Corresponding to the pathology diagnoses, the ultrasound imaging characteristics including beginning enhancement time, the peak enhancement time, and the enhancement strength on benign, malignant lesions, or normal pancreatic parenchyma were evaluated. [Results]Pathology diagnoses of 57 malignant lesions and 23 benign lesions were made on surgical or needle core biopsy specimens. CEUS and conventional ultrasound have diagnostic sensitivity (88.71% vs 73.81%, X2 =3.884, P =0.049) specificity respectively (88.89% vs 68.42%, X2 = 2.729, P = 0.099) accuracy respectively (88.75% vs 55.00%, ;(2 = 23.921, P =0.000) positive predictive value was (96.49% vs 54.39~, X2 =27.269, P =0.000) and negative predictive values were (69.56 Mvs 52.17 %, ;(2 = 1.460, P = 0.227). The enhancement started later in malignant lesions than normal parenchyma ( P 〈0.05) enhacement subsided earlier in malignant lesions than normal pancreatic parenehyma the duration time in malignant lesions is shorter than that in benign lesions or normal parenchyma ( P 〈0.05). [Conclusion]As a safe, easily to perform, non-invasive diagnostic method, CEUS has a high diagnostic value and should be wildly used and applied on SFPLs.
