调强放疗同期顺铂时辰化疗治疗局部晚期鼻咽癌疗效观察  

The Curative Effect on IMRT plus Cisplatin Chronochemotherapy in the Treatment of Locally Advanced Nasopharyngeal Carcinoma

摘　　要：[目的】探讨调强放疗同期顺铂时辰化疗治疗局部晚期鼻咽癌的临床应用价值。【方法】将120例局部晚期鼻咽癌患者随机分为观察组与对照组，每组各60例；观察组采取调强放疗同期顺铂时辰化疗治疗，对照组采取调强放疗同期顺铂常规化疗治疗，比较两组患者的近期疗效及毒副反应情况。【结果】化疗3个周期后，观察纽有效率（RR）为85．00％，对照组为76．67％，两组比较差异无统计学意义（P〉0．05）；两组患者的放射性皮炎发生率、黏膜炎发生率比较无统计学差异（P〉0．05），观察组白细胞减少、血小板减少及恶心、呕吐发生率均明显低于对照组，且毒副反应程度明显轻于对照组，两组比较差异显著（P〈0．05）。【结论】调强放疗同期顺铂时辰化疗治疗局部晚期鼻咽癌相对于常规化疗可有效减少化疗药物的毒副反应，提高患者耐受性，值得临床推广应用。[Objective]To investigate the clinical value of IMRT （Intensity-modulated radiation therapy） plus cisplatin chronochemotherapy for locally advanced nasopharyngeal carcinoma. [Methods]120 patients with locally advanced nasopharyngeal carcinoma were randomly divided into two groups, 60 cases in each. The observation group were given IMRT with cisplatin chronochemotherapy ,while the control group were given IMRT with conventional cisplatin chemotherapy treatment. Efficacy and toxic side effects of the two groups were compared. [Results]Three cycles after chemotherapy, RR of the observation group was 85.00%, that of the control group was 76.67%, the difference was not statistically significant （ P 〉0.05） In the patients of the two groups , the radioactive dermatitis incidence, the incidence of mucositis have no statistical difference （P 〉 0.05）, leukopenia, thr0mbocytopenia, and the incidence of nausea and vomiting of the observation group were significantly lower than those of the control group, and the adverse reaction was significantly lighter than that of the control group, the two groups are significant different （ P 〈0.05）. [Conclusion]Intensity-modulated radiation therapy plus cisplatin chronochemotherapy in the treatment of locally advanced nasopharyngeal carcinoma can reduce the side reaction of chemotherapy drugs more effectively than the conventional chemotherapy , and improve patients＂ tolerance. It is worthy of clinical popularization and application.
