腹腔镜手术治疗胆囊管结石62例体会

摘　　要：【目的】探讨胆囊管结石的诊断及腹腔镜手术治疗方法。【方法】对2008年2月至2013年1月石家庄市第一医院外科诊治的62例胆囊管结石行腹腔镜胆囊切除术的临床资料进行回顾性分析。【结果】术前超声、CT共诊断胆囊管结石42例，诊断率分别为29．0％、69．1％。顺利完成腹腔镜胆囊切除术（LC）60例，2例中转开腹。11例行术中胆道造影发现胆总管结石2例，分别行腹腔镜胆总管探查取石、ERCP治疗，均痊愈出院。【结论】结合超声、CT检查及术中仔细探查可避免遗漏胆囊管结石，选择性术中胆道造影检查有利于胆总管结石的诊断。胆囊管结石实施腹腔镜手术治疗安全、可靠。术中灵活应用LC的处理技巧是腹腔镜胆囊管结石手术成功的关键。[Objective]To explore the diagnosis and laparoscopic surgery therapy for calculus of cystic duct. [Methods]The clinical data of 62 cases of laparoscopic cholecystectomy in the First Hospital of Shijiazbuang from February 2008 to January 2013 were retrospectively analyzed. [Results]Preoperative B ultrasound and CT were diagnosed with 42 cases,of cystic duct stones, the diagnosis rate was 290 and 69.1% respectively. 60 cases of laparoscopic cholecystectomy （LC） were successfully completed, 2 cases were converted to open surgery. 2 cases of common bile duct stones were found in 11 cases, Laparoscopic common bile duct exploration and ERCP therapy were treated respectively, All patients were cured and discharged.[Conclusion]B-ultrasound, CT combined careful exploration in operation could reduce omission of calculus of cystic duct. Selective intraoperative cholangiography can help to diagnose calculus of common bile duct. Laparoscopic operation for the treatment of cystic duct stones is safe and reliable. Flexible application of LC in the treatment is the key to the success of laparoscopic cholecystectomy.
